I'm far from LLMD and reliant upon insurance. My neuro ran an immunoblot and the results are below. My cortisol is also elevated at 23
This test was negative - LYME DISEASE AB W/REFL TO BLOT (IGG, IGM) <0.90

I see my primary next week. The best research hospital around has infectious disease, but will not take a referral without a positive western blot.

Looking for suggestions on tests to request from my primary. He is pretty good about testing what I ask for. He is just as curious about finding out what is wrong with me as I am for the most part. I have all the symptoms. A full work up two years ago found celiac, but neuro symptoms keep returning. The fatigue and joint pain are unbearable right now. I was csf and seuroneg for lyme 2 years ago. I know that doesn't mean I didn't have it then as well. I've had knee pain and swelling off and on for 18 years, since age 15. Tested for MS as 19, then again at 31, and again last month.

I'm excited that something showed positive and terrified at the same time. Answers provide hope that I can one day feel energy again, and hopefully be in less pain.

Hi Kalymika, Welcome!

Although your Immunoblot was a negative overall, you had a positive Lyme specific band (23) show up for IgM

A good LLMD will not rely on a positive test result.

Here's what Dr. H. (well-known and one of the best LLMD's ) said in an interview:

"We need to teach doctors how to make a clinical diagnosis of Lyme disease after ruling out other diseases, supporting that diagnosis by finding borrelia-specific bands on the Western Blot, which show evidence of exposure. This would include finding the 23(OspC), 31(OspA), 34(OspB), 39, and 83-93 kDa bands on the Western Blot. If the patient has classic Lyme symptoms such as migrating joint and muscle pain, with good days and bad days, and symptoms that come and go, made better or worse with antibiotics, you need to use your clinical judgment and recognize that those are classic Lyme symptoms. "

Your cortisol is 23? What is the range for it?

"The best research hospital around has infectious disease, but will not take a referral without a positive western blot. "

I doubt if that would be a good place for you to go for treatment anyway. You want a Lyme Literate Dr. (LLMD) as most ID Dr.'s will not treat chronic lyme with the appropriate combinations and dosages of antibiotics and not for long enough time.

The fact that a positive WB is required is a strong indicator that this isn't a good place to go.

I'm sorry you have been dealing with this for so many years...but hopefully you are on the right path now.

(MS is one of the misdiagnoses in Lyme patients.)

Have you been tested for Ehrlichiosis?

I'm asking because band 45 is sometimes positive when people have Ehrlichiosis.

It also matches some of your symptoms:

I have not been tested for any coinfections yet.

The cortisol range is 7-22

My mpv on my CBC is also elevated at 13.5, high range 12. This commonly is elevated for me.

My tsh went from 1.76 to .47 between Jan 9 and 2/23, same collection time. Range is .40 to 4.5, but that seems like a pretty hefty drop.

Thx for the link on bands. I've been looking for that info.

Praying my primary will consider it positive since I can't afford a Lyme literate doctor. Going to try to see if any from the reference post on the forum take my insurance.
