Trimethoprim - Herx or Adverse Reaction?

I've had a reaction to trimethoprim which was very similar to how Bactrim (sulfamethoprim + trimethoprim) affected me. My doctor put me on trimethoprim alone to test whether the reaction to Bactrim was a sulfa allergy.

Days 1-4: 100 mg. Pressure around eyes, lymph nodes start to mildly ache, brief small lights in vision.
Days 5-7: 100 mg BID. Evening restlessness, pins and needles in webbing of fingers, muscle twitching, peripheral neuropathy flare, cognitive impairment.

After my second dose on the 7th day the symptoms became severe. There was migrating pain in joints including wrists, mostly knees; migrating burning pain in muscles; headache like I had been hit in the head; sinus pressure; migrating pain in tendons; strong tingling in areas of existing neuropathy; raw pains in cheeks and mouth (this made me worry about Stevens Johnson).

I skipped my morning dose, and all of the symptoms have gotten much better over the last 15 hours. I've been diagnosed with Lyme, bart, and babs. What puzzles me is that I've taken augmentin for a sinus infection and had no issues with my knees. I think I'm going to ask my doctor to switch me to another class of antibiotic if this wasn't definitely a herx.

My detox is Kudzu root, 250 mg ALA BID, 600 mg NAC BID, Pinella/Burbur, epsom baths, serrapeptase, and vitamin C; I'm also on Buhner herbs. I'll update this thread tomorrow after I consult with my doc.

I'm not sure if what I'm experiencing is from the Bactrim or not...not sure what to do.

Have you tried to take a binder during this to see if that helps a bit.

To me it sounds like herxing because so many different symptoms and intensity like we get from the infections.

I found this about Sulfa reactions:
symptoms of sulfa allergy are similar to those of other drug allergies. Symptoms may include:

skin rash or hives
itchy eyes
itchy skin
congestion
swelling of the mouth
swelling of the throat

Hope this helps a bit and hopefully others with more experience with sulfa reactions will see this when post is bumped up.

My doc thinks the infection is too embedded for me to take antibiotics. I've had Lyme symptoms for over 25 years, bart about 20 and babs 10. I'm going to add MC-BAR-2 to my Buhner protocol, and look into infrared sauna and HBOT to get my body in better condition.

Is that a LLMD saying that? Or an ND?

I’m thinking ND - Doesn’t sound like something a LLMD would say

LLMD, that wasn't an exact quote. My first treatment was primarily for babs and Lyme, I was satisfied with the results and left earlier than they recommended. They wanted me to treat bart but I didn't think I had the symptoms, 3 years went by before I had to go back for neuro issues (bart!). I've had improvements on 6 months of herbals, the doc thinks I should continue on them until I plateau.

After sleeping on this I realized I was ignoring the obvious: the joint pains flare up with bart symptoms, only when taking bart antimicrobials, therefore I should conclude that bart (not Lyme) is the cause. Pubmed has a hit for arthralgia and bart, Musculoskeletal manifestations of cat scratch disease:

Musculoskeletal manifestations (MMs) of bart (CSD) are more common than previously thought and affect one-tenth of patients with bart. MMs occur mostly in patients aged >20 years and may be severe and prolonged. Osteomyelitis, the most well known MMs of CSD is, in fact, the rarest.

We conclude that myalgia, arthralgia, arthritis, tendinitis, and neuralgia are important yet unrecognized MMs of CSD and should be included among the growing list of atypical CSD manifestations caused by B. henselae. These manifestations are often severe, with a protracted course, and may evolve into chronic conditions. Familiarization with CSD-associated MMs by general practitioners and orthopedic, rheumatology, and infectious diseases specialists is desired to provide correct diagnoses for patients with such MMs and to better characterize and study CSD.
