I understand that you need to add multiple antibiotics when treating these bugs, but some people are very sensitive, and will need time to build up to it. Yes, you do need to be careful with causing bug resistance, but it usually takes some time for that to happen.
Increasing slowly over time usually helps someone with extreme sensitivities and/or Herxes, especially if they have really bad anxiety.

starting lower doses sometimes - but I wasn’t talking about
that - I am asking if there are plans to add something to the Azithromycin... as it isn’t effective to treat Babesia.
An anti parasitic/anti malarial is required.
