Substernal/retrosternal burning, bartonella related?

Hi all,

Over the last few days, especially the last 24 hours, I’ve developed some alarming symptoms and am curious as to what to do.

I’m having burning sensations behind my sternum and at the xyphoid process area that seems similar to heartburn but doesn’t seem to follow any particular pattern. Along with this, I’m having some diffuse tingling and burning sensations in my feet, legs, and arms that come and go. During the middle of the night I had a brief episode where it would get worse with inspiration but that subsided (I experienced this one another time maybe a month ago). Of course, I am most concerned about any cardiac related issues vs. Bart herx vs. acid reflux/heartburn or something else. For the entire month of March I wore a halter monitor due to concerns of POTS/tachycardia but never heard back any negative results from that (my LLmD is pretty sure I have POTS). I have no other history of cardiac issues. I have had diarrhea for about 5 days now but no fever or adominal pain/cramping. Only other symptom is some dizziness over the past 2 days.

My Medications are as follows:
Minocycline 100mg 2x/day
Rifampin 300mg 2x/day
Clonazepam .5mg 4x/day
Supplements:
Magnesium 200mg 2x/day
Glutathione 250mg 2x/day
NAC 900mg 2x/day
B vitamin complex 1x/day
Omega 3 1x/day
Probiotic 1x/day
5-HTP 100mg 2x/day
Curcumin 500mg 2x/day
Alka seltzer gold 1x/day

I’ve been on the Rifampin for 2.5 weeks now (first week half dose) and Minocycline for about 5-6 weeks. I haven’t skipped any doses but I’m planning on holding the rifampin until tonight at the earliest, am still debating whether to take Mino regularly today (for it’s anti-inflammatory properties) or hold.

Does this sound like a Bart herx? Perhaps it’s toxins in my system causing these issues? Could my liver enzymes be up? (They were perfect 3 weeks ago). Heartburn/gastric irritation? Anxiety? Or cardiac? My wife wants me to go to urgent care but I’ve held off so far, I worked about 2 hours this morning (where the symptoms had subsided) but came home after I could feel the sternal burning creeping back in.

I did do some yard work and got out in sun/warm weather on Thursday and Friday, and even managed 12 minutes on the elliptical yesterday despite not feeling great.


Any advice is appreciated!

It certainly could be bartonella.
Barontella is known to hit the ribs and the sternum is connected to the ribs, right?


Does it feel like heartburn, rather than structural pain?

Yes it feels more like heartburn. I went to urgent care, they did an EKG which was fine and prescribed me protonix for what they dx as acid reflux. Hoping that does the trick.
Sx began Aug 2017.
Dx with Lyme 10/13/17. Rx with Doxy/Zith/Tindamax x 2-3 months (80% improvement)
Dx with Babesia Duncani 1/2018. Rx with Mepron/Zith/Doxy x 1 month
2 week break due to worst herx/experience of my life
Coartem/Zith/Doxy 2/2018-3/2018 (60% improvement)
Coartem/Zith/Mino beginning 3/30/18
Dx with Bartonella 4/7/18

It sounds like a bart herx to me. Bart gave me silent reflux symptoms and IBS-C. It can cause all manner of GI symptoms, and they can rotate. I've also had a few chest pains. Wasn't my heart. Pretty sure it was ole bart.

I’m probably going to do a half dose of rifampin for the next day or 2 and see how it goes. It was pretty scary, I’m thinking herx too due to the weird tingling sensations on/off, but you can never be too careful. I don’t see my LLmD until next Thursday so I’m hoping this resolves.

How soon into your treatment did you get the reflux symptoms?

I had the reflux symptoms before I knew about the Lyme. Had it for 8-12 months. Took reflux meds for 6 months and weaned myself off. All before Lyme diagnosis.

Had a flareup of the reflux a few weeks ago. Lasted about a week to 10 days. I rarely have flareups of reflux. I think it was either a herx or some other gut pathogen. I had diarrhea and vomiting and gastritis one day, and the reflux flareup followed that.
