These are to be taken to your physician/neurologist/headache specialist and filled out and SIGNED by the doctor with what they want you to be given in the event that you need to go to the Emergency Room or Urgent Treatment Center.
 
These forms are to be filled out by YOU, with your patient registration information, what your pain rating is on a scale of 1-10, what medications you have already taken etc, this helps because when you are in severe pain-enough to go to the hospital you will probably not be in the condition to answer all of these questions.
 
This is another link for a form that you can take to your doctor-again to be filled out and SIGNED by the physician for what medications you are on, and what is to be given to you in an Emergency situation.
 
(Credit to where it's due, the first two are from one of the greatest headache advocates-Teri Robert...She's the best and has a new book coming out in November, which I can not wait to review...and the last is from the National Headache Foundation, who is always working to keep us, headache sufferers in the know.)

I love, especially the one from Teri, because it say that #1 Migraines are a NEUROLOGICAL DISEASE...and #2 we are not "drug seekers" and that "It is often very difficult for Migraine patients to receive adequate care offered with dignity and respect because of others who go to emergency departments and after-hours care facilities, feigning symptoms to obtain narcotics. I assure you such is NOT the case with this patient."

The first time I read this piece of paper I had been through one of the worst ER visits of my life...and I could have kissed her for creating this document!

I, of course, stress to people not to abuse this document and keep ER visits to a minimum and blah blah blah...but we are all here for legitimate purposes...and that is like "preaching to the choir"

Ha ha...I feel like I am "talking" to myself in this thread!

No, I just found an interesting site...its www.myheadachediary.com you have to register to use it, but when you have a headache you input some information like the date and time, what type of headache you have, how much sleep you had the day prior to the headache, what you ate before the onset, pain intensity, warning signs and symptoms, possible triggers, treatment and how the treatment worked on a 1-10 scale, you then submit the information and it calculates the data.

I created a few test entries so I could see what the reports look like. They are great. They show pie charts with what  percentage is highest for each...for example it said that 66% of the time irregular sleep patterns trigger migraines for me and that 100% of the time fatigue and mood changes were signs of a migraine to come...

I think the only INCONVENIENT thing is that I don't feel like being on the computer most of the time when I am in the midst of the a migraine!

These are great Sara! I'm going to put the Headache Diaries in a separate post so people can find them easily. They're so important.

Those forms are fantastic. Thank you so much for posting them. Wouldn't it feel wonderful to go to an ER and not be treated like dirt?!

That's exactly the point!!!! Thanks for the post!!! I have used these before and they do serve the purpose...at least the staff knows that you have done your homework and you're not just a junkie!

Here are the forms you guys...

Take this to your DR to fill out so when/if you have to make a trip to the ER they have written out the course of treatment they want prescribed for you!!!

Hi Sara

I can't seem to open these files, but I would LOVE a form to use for the ER.  I am tired of the ER staff rolling their eyes and telling me to simply try toradol.  confused

Can you maybe re-post them or email them to me?  Thank you so much!  Everything you are doing here means more than you can ever imagine!!!

I do my best and sometimes wish there was more I could do!!!

I have been fighting with those links today to get them to work...I won!!! I have just added the ER Form at the bottom of my signature click "VIEW IMAGE" under ER Form and then you can print that off, save it, or copy and paste it to whatever you would like...you could even email it to your doctor. Whatever is easier!

I figure this way, it is always there and we won't have to keep bumping that thread to the top too tongue

This is awesome. My neuro wants me to keep a diary. A calendar just does not cut it.

I think a diary is better too...I need more space, I write too fat and have way too much to say~
 
I always include how I feel, emotions and all! Sometimes that helps me remember the specific migraine episode. I think again this is one of those things that is specific to each person...
 
There are a lot of great ideas online though if you need help kicking a migraine diary/journal off!

jsut wanted to say that the form was amazing. The doctors at the ER always just do what it says, and even make a copy and keep it on file. My only prob now is my doctor refuses to prescribe or suggest narcotics. He is the best Neuro. I have ever seen (I've seen like 10, litterally) and I have actually had great success with him, but he doesn't give me anything for breakthrough...so my PC.Doc gives me Stadol NS once every 3 months, and I still have a lot of ER visits. Most docs. know me by now, but everyonce and I while I get that one doctor who "knows everything" and insisit on Torodol or something. One time I got a shot for continuous vomitting(it wasn't phenergan) and it helped actually, but made my legs cramp so bad. And I can't remember the name! Just thought I share that the form does work and the ER staff treats you a lot better with it. THANKS!!

We use the word 'bump' as something to put on the post to bring it back up to the top for people to notice. i.e. 'bump' it up!!
