What type of headache is this?

I have had migraines since I was 13.l am now 37 so this is not a new thing for me. I have been taking nortriptyline 100 mg at bedtime as a preventative. I have been having these head pains on the top of my head like intense pressure.. it literally feels as if my head is about to explode. My normal migraines have always been on the right temple and include my right eye and nausea and light sensitivity.
this is a new type of headache for me, but feels like a migraine, just in the wrong area for me..

I saw my neuro last week and he upped my Nortryptline from 75 mg to 100. I had been taking 75mg for about 2 months and was still having to go to the ER at least 3 times a month. Could this be causing my pain? None of my typical migraine meds work... I have axert, imitrex nasal spray, imitrex shots, florinal and caffergot and samples of relpax. None of these seem to help, and I have had a hedache every day this week. i have also had a stiff neck all week.

Has anyone experienced anything like this?

What do you think iis the cause?

Any help at all would be appreciated.

Hi Tracy, sorry to hear that you have been suffering with your headaches. Did you tell your Neuro last week about the unusual pain on the top of your head? If you didn't I suggest you try and contact him again. Here in the uk my way of doing this would be to phone the Neuro's secretary and explain the situation and ask for their advice and help to make a speedy return visit.

I had my first migraine at 17 and I am now 52. Thankfully my migraines respond to imitrex most of the time as an abortive therapy but sadly not all the time. My pain is predominantly on the left in the corner of my left eye and behind it, and then it radiates down the side of my head till it stops at the centre of the back of my head. Sometimes it comes on the right side doing something similar but not quite the same, and sometimes it can be deep in the centre of my head, and this is usually when I vomit the most!

My migraines have evolved over the years to what they are now, it may be that is what yours are doing but of course I am not a Doctor so I can only speculate. I think it is important that you talk to a Neuro/Doctor about this. All the best, take care Ann.

Hi Tracy,
Things do seem unbearable. I also have had periods when my migraines seemed to change presentation. I was involved in a car accident, got some physiotherapy that made me feel better. But eventually I got tension in my neck so bad my physician recommended physio again. That did not do it but massage therapy solved the problem. That was 3 years ago and I no longer get pain in my neck. If it helps you : good.
Do both your neuro and physician know all the symptomatic drugs you're taking ? Recently mine cut everything out, leaving me with 1 preventative medication, 1 first line treatment and 1 second line treatment. He felt I was taking too many therapeutic agents.
