the best suggestion I have is more long term...get a better doc to treat you in the long term so that when this type of situation comes up you have a better "team" who is willing to offer the "big guns"
I run into the same situation...I am in pain, you don't want to help me...but I don't want to come out and ask for specifics because you will think I am a druggie...
well, if I/we were...we would be on the corner selling more than our belongings...

We would not be sitting in an office calmly talking about things or asking to be treated for legit pain. Take someone with you, someone who has seen and can honestly and rationally discuss your situation also. Realize your doctors standpoint as well, they have pressure from authorities to be careful about the amount and the patients they prescribe narcotics to...Take your medical records with you, take your diary...take as much "ammo" with you to back up your "story"...present your case like a lawyer would. Try to leave out emotion, as hard as that is. Let them see the facts, sometimes doctors get hard to the feelings because they see so much day in and day out and we have to prove the case and get them to see why as far as reasoning...
I know that is more than we are up to, but in the end it is about what we need to do for ourselves and our wellbeing!
Hang in there...
and in the end, find a doctor who works as hard for you as you work for yourself.
