Has anyone who has their rectum and has experienced rectal discharge had it slip out accidently? Does this mean problems for the jpouch?

No, this is totally normal with a loop ostomy and is not indicative of jpouch function.

My permanent ileo was done 3 1/2 years ago. I have some rectum and my anus. I still have some yellowish discharge on occasion. No urgency. Surgeon said it is normal.

Had it for little while after sugery then it stopped after a bit, no worries
