Flange Leak often

Hi, Now it is one year after my husband had to go in for a ileostomy. Still we cannot manage the ostomy well. After 3 days, the flange gets crimpled up and fluid comes out disturbing the gum causing leaks. In Sri Lanka the flanges are quite expensive and we try to to keep it on for at least 7 days but since late it cannot be done due to this problem. Noticed that my husbands ostomy is not a perfect round. towards the (his) left side of the ostomy, there is an oval shaped buldge .....a bit of intestine I think because it is pink in colour, so when I cut the flange, however much I try to see that it does not rest on the part of intestine, it does rest on the buldging part and not on the skin itself. The other area of the flange rests on the skin. My thinking is that because part of the flange is resting on the intestine which is slimy, the gum gets dislodged and causes leaks. Observed that the leak is 'always' from that point only. Anyway that was how the hospital staff had placed it too after the surgery. From the time he was discharged we were having this problem of leakage and changing flanges often and no one could advise us on what is normal and what is not. Note this is a 'looped' ileostomy. We do not have WOCN is Sri Lanka and most people are not aware of this subject. Even the Emergency Medical Team whom we have engaged just change the flange and bag in the same manner and leave without any discussions. What I am doing for the past year is with interaction from forums and suppliers on the internet. I need to know whether this is NORMAL in this type of surgery or there is a solution. Or whether I am doing it correctly or NOT. Thank you very much for your care and concern. Best regards, Mary Anne idea

I would on occasion use medical grade tape to reinforce the flange and my skin always held up and the flanges lasted longer. It's possibly worth a try or try a different flange brand all together.

The tape is a lot cheaper then extra bag changes :p

One thing I use that helps keep everything stuck onto me is Tincture of Benzoin. A lot of people on this board use it for different things. I have problems with the outer part of the flange sticking, since it's near my belly button and the skin curves there. So I brush the Tincture of Benzoin on that area, wait a minute for it to dry, and it provides enough stickiness to hold on to the flange. It also is a very good skin protectant. If you are able to obtain it (you can get it from Amazon.com and other places online), it is cheap (about $10 American) because one bottle lasts a very long time.

Hi Mary Anne. I would agree that your husbands problem is the flange sitting on the bulge of his stoma. the flange definitely needs to be against the skin. if part of it sits on the slimy stoma it will leave a gap underneath allowing output to easily get into the gap and work its way to the outer edge.

The only thing i can suggest (without seeing his stoma) is getting the hole large enough that it sits on his skin and not the bulge. but you will almost certainly need extra protection because you will probably leave exposed skin by making it larger. you would need to fill any exposed areas with either a stoma paste or rings.

again it's really tough to offer any real suggestions as everyones stoma is completely different and the best way to solve problems is by seeing it. And the problem with forums is we usually only offer the solution we had to our own stoma problems and not necessarily the solution your husband needs.

I have a similar problem before too, Leaked all the time. My supplier suggests me to try (ConvaTec EakinĀ® Cohesive Seal) to cover the uneven area. It worked very well. I found that Ebay is great source of inexpensive and excellent supplies.

Have you tried a convex wafer? They are more solid than the flat ones, and they stick better. They may also push the intestine out a bit and away from the skin so that you can get a better seal (I can't quite picture how your husband's stoma looks).

If you can call to the U.S., the company convatec has a lot of nurses on the staff. They are very good and could help you over the phone.
