All parts of the GI tract generally demonstrate a low level of controlled inflammation -- it's not pathological and it can be seen in all patients regardless of IBD status. This has always been my understanding of the "all pouches have mild inflammation" conventional wisdom.
That said, it sounds like your surgeon has a bee in his bonnet about
limiting antibiotic use. It's insane that you haven't been treated with empirical antibiotics yet. It's just cruel to keep you waiting for biopsies to come back.
