Orgasm difference pre and post Da Vinci Prostectomy

     I had Da Vinci Prostatectomy on 4-17-09 at UCLA by a top doc.  Both nerves were spared and as of July my first Psa was less than 0.01.  I have been using Bimix  with a 1/2 inch needle and .35 for good results (if I hit the area right).  Several times I have had little or no results.  But 80% of the time, I hit gold.  I am still trying to figure out why I loose out on 20% of the time.  Anyway, the orgasm is not as intense as it was prior to surgery.  If I had to put a number on it it is perhaps 70% of what it was prior to surgery.  Does it get better???
 
    Also, since I am only 6 months post surgery, how do you know when an erection starts to come back naturally and when.  Is it something that happens all of a sudden, or does it progress over time.  Are there any signs.  HELP!!!

Several of us have found that using what we call a pump plumping technique can get the percentage up to almost 100%. You use a penis pump and penis ring. Pump up about 1/4 or so, just to a little plump, apply the penis ring and then inject. The pump pulls blood into the cavities and expands the walls of the cavities, the ring holds the blood- and drug- in the cavities. From there, a gentle massage to spread the drug out and hold in the blood and drug mixture for 3 to 5 minutes will allow for expansion of the cavities, allowing for a more accurate needle hit and the adsorption of the drugs, rather than letting it run back out of the penis during normal blood exit. Inject halfway from the base and the head of the penis, at the 3 and 9 oclock positions.

REcovery of function can take 2 years or more. I think most peoples experience is a slow improvement of function. At 6 months, you still got lots of time to go and there's plenty of time to recover. I am at over 2 years, and still not recovered, so it can take a long time, and sadly sometimes it may never come back. Wait a while before getting too concerned about it.

Mike, welcome to HW, you got some good questions there.

I can honestly say, it took some time to get use to dry orgasms, but once I did, they continued to build up in both intensity and duration. I would say what I feel is 125% better than before, if you dont factor in the "wet" issue, which I no longer miss.

You will find such a variation of answers and results here about the ED issues. James, above, is a good example of having a nerve sparing operation, but still having issues 2 years out. I am on the other end of the spectrum, only one nerve spared (right side), and haven't had any ED problem right from the start. It varies that much.

In the beginning, it just plain sucked but after the first year the orgasms have differently exceeded the days of the juice. They seem to last longer and be more intense. LOL, no fighting on who has to sleep on the wet spot.
12/06/07 DaVinci and open prostate surgery after difficulties in breathing stopped the davinci.

Dry feels better in my book, dry also means no recovery , still stands tall and ready for the next round no rest time needed now

I am approaching three years post DaVinci surgery (34 months).  My orgasms were very bad for the first year, but have improved over time. Still not anywhere near pre surgery.  Sometimes it is just a quiver and then too sensitive to continue, sometimes pretty good. The intensity of stimulation makes a difference, less during intercourse and more with manual stimulation.  That is how we often finish.

I successfully use bimix with 100% results using about .25.  I had excellent pre surgery performance, but I think age is a real factor.  I am unable to get a usable erection without the bimix injections.

I had Da Vinci surgery on Sept. 17th. I have been one of the extremely lucky guys here that can get an erection without using a pump. It takes 100mg of Viagra or 20mg of Levitra and a lot more time than it used to but I still feel blessed. I do not mind the dry results but I can say that the feeling is not near what it used to be when it happens now. Maybe it will change over time. If not, I'll take less intense feeling over less hardness any day.:). Wes

You will start getting nocturnal erections when it is getting better. Now, just cause you get an erection at night or in early AM, does not mean you are out of the woods, but that is a crucial sign. For me, the morning wood has gone completely. But with a lot of work I can get an erection - rarely usable without drugs - it goes away too fast. Also, how norny you are feeling has a lot to do with it. I'm afraid many of us are suffering performance anxiety and not even realizing it. Try viagra at night. See if you get erections.

Rarely do the orgasms during intercourse become anything like they were. Now, with manual, they are very intense - much more than before and longer. There is some solace in that!

One thing I wanted to add, was that has been talked about here on and off, I am now 11 1/2 months out of surgery, and although I was lucky and didnt have any ED problems in obtaining "wood", things are different of course. Up until recently, I could not "think" myself hard, no matter what. I just assumed that was another casualty of having sex after PC surgery. In the past few weeks, started having erotic dreams (something I am not accustomed to) and finding myself hard upon wakening. Now, if I am not too tired or wore out, I can achieve hardness by thinking of something "pleasant". Doesn't work every time, but it is starting to work again. I only mention it to give hope to our brothers here, that time heals for many. I didn't think this part was ever going to happen again, but finding myself surprised.

You guys with dry orgasms, you don't know how very lucky you are. I would give anything to be able to have a dry orgasm.  Since my surgery in 9/2007, whenever I have an orgasm now, I will ejeculate urine.  Matter of fact, I ejeculate urine with more force now without my prostate than I ejeculated seman with my prostate.  With a good intense orgasm, I can hit the ceiling when I ejeculate.
 
I have tried  to empty my bladder before sex, but I will still ejeculate some urine with orgasm and drip during foreplay.  The doc says urine is steril and will not hurt your partner if you get some in her, but just try telling that to a woman. 
 
I tried using a condom, but then I can not maintain a erection because I can not feel anything. You don't know how frustrating this is.   I was very lucky to have no ED issues after my surgery, and I can get a pretty rock solid erection without any drugs, but as soon as I get aroused, I can actually feel my splinkter muscle start to relax and I will start to leak.  I have to cover the bed in towels so I don't wet the sheets.  I also have to be very carefull during foreplay so I don't drip on my partner. Taking all these precautions take all the fun out of sex now.   Boy, what I would give to have dry orgasms.

Surgery 9/2007 Uni of Rochester Strong Memorial NY with Dr. Jean Joseph (1300 plus surgeries)
 
Path - Negative margins, cancer in 20% examined tissue, Gleason 6
 
Post Op - No ED issues, full erections without drugs,  used 5-7 pads a day for 3 months. Now dry except for stress leaks now and then.
 
All post op psa's <.04
