Rising PSA after Surgary and Radiation

Hello, I live in Canada and Greece. I’m 66 years old and in excellent health and physical condition. I’ve been on a very healthy diet for decades and haven’t eaten meat since 1992; instead I eat fish. I run and exercise almost every day and am also very active with my business.

 

I had a surgery in January 2006 and IMRT Radiation in June 2008 when my PSA was 0.08, which were both done at The Cleveland Clinic.  I could have done both in Canada at no charge.  I decided to have the surgery done in Cleveland as i thought they had better equipment at that time and doctors and the money wasn’t an issue for me.

 

 Diagnosed   Tc1 PSA 12.2 Gleason 3+4     12 Dec 2005  

Surgery   Cleveland clinic                              16 Jan    2006 

Final pathologic staging T3a N0 M0 Gleason 3+4 one 2mm max positive margin right side.

I’m not sure if surgery was my best option.  

One month after surgery my PSA went down to undetectable levels and it stayed undetectable until Nov 2007 then started rising and when it got to 0.08 on 3 of June 2008, I started 35 IMRT treatments of 2.0 Gy   70.0 Gy total and finished  24 Jul 2008. I was told that my chances of cure were very high at 85%.

Two months after radiation 17 Sept 2008 my PSA went down to 0.03 and four months later in January of 2009 it went to 0.06.  The latest test on 29 of April 2010 is 0.32. I was told by the doctors in Cleveland and Canada not to do anything for now and do another test in Sept 2010.

Although my spirit is still high and I still enjoy living, I am disappointed and confused  that with my chances being so high that I wasn’t cured.

I would appreciate any advice from anyone as to what to-do next; if there are any natural supplements that could help or any new treatments, new vaccines or any information from anyone with a similar condition.

I wont try to answer your question as they are many more on this forum with much more expertise and wisdom than myself. Just wanted to welcome you, and it sounds from a lifestyle perspective you are doing a lot of things right. Keep us posted and my best to you.

Starting IMRT on 1/18/10, Completed 39 tx at 70 gys on 3/12/10
6 week Post IMRT PSA .44 a drop from .5 but maybe more

My best advice is to see one of the top prostate oncologists, either Charles Myers in Va, Mark Scholz in Marina Del Rey Ca or go on prostate pointers p2p and ask Dr Stephen Strum. Your stats are on the borderline and only a specialist who has seen thousands of cases of reoccurrances can make a reliable DX and suggest future steps.

64 years old.

PSA rising for 10 years to 40, free psa 10-15. Had 5 urologists, 12 biopsies and MRIS all neg. Doctors DXed BPH and continue to get biopsies yearly. 13th biopsy positive in 10-08, 2 cores of 25, G6 less than 5%. Scheduled for surgery as recommended by Urological Oncologist.

2nd Opinion from Dr Sholtz, a Prostate Oncologist, said DX wrong, pathology shows indolant cancer, but psa history indicates large cancer or metastasis. Futher tests and Color Doppler confirmed large transition zone tumor that 13 biopsies and MRIS missed. G7, 4+3, approx 16mmX18mm.

Combidex MRI in Holland eliminated lymphnode mets. Casodex and Proscar reduced psa to 0.6 and prostate from 60mm to 32mm. Changed diet, no meat and dairy. All staging tests indicate that tumor is local and non agressive. (PAP, PCA3, MRIS, Color Doppler, Combidex, tumor reaction to diet and Casodex, and tumor location in transition zone). Surgery a poor option because tumor is located next to the urethea and positive margin is very likely; permanent incontenance is also high probability with surgery.

Seed implants on 5-19-09, 3 hours door to door, no pain, minor side affects are frequency and urgency; very controlable with Flowmax and lasted 4 weeks. Daily activities resumed day after implants with no restrictions. Gold markers implanted with seeds to guide IMRT.

25 treatments of IMRT 6 weeks after seed implants. No side affects at all.

PSA at end of treatment 0.02 mostly the result of Casodex. When I stop Casodex next week expect PSA to rise. Next PSA in November. Treatments and side affects have greatly exceeded my expectations. Glad to have this 11 year journey finally conclude.

George there has been no scientific research that reports supplements are effective to my knowledge. But by the same token if you think it will work, it just might.

George, I don't know about Dr Strum, but Dr Myers strongly believes in diet and suppliments. You should read his book. I know advocates taking pomeganet, lycopene, and vitamin D. BB

We just got an appointment with Dr. Myers for late August/Sept. Call now and you should get an appointment around that time. We just read his book and very informative! Vit D, Vit C, Zinc, Lycopene (from watermelon especially), flaxseeds, selenium are mentioned in various books and sites. Go to a good vitamin store to get your supplements. Also, no meat or white flour is good. Dr. Myers recommends the Mediterranean diet as you are on. You should meet with an experienced medical oncologist. Possible for you to get to NY? I wish you the best!

Thank you Michael and Hope and Prayer. I’m drinking 100% 250 ml of Pomegranate juice and eat approx 300 ml of Blueberries every day for over 5 years and last month I started taking 2000 IU/UI vitamin D3, calcium and 2000 mg of AHCC+ and Omega 3, 3rd party tested. Not sure if this will help an, I don’t think that they can do harm. Although I eat a lot of vegetables and fruits I will try Lycopene and will try to get an appointment with Dr Myers. I will try anything that is not harmful to prevent having to start ADT. Have a nice day and also wish you the best. George

I hope you can get an appt just as quickly. After the initial phone interview, Dr. Myers will want all your test results so good to get them in order now and ready to forward to him. We were told that he will meet with you for 2 hrs at your appt which is unheard of! You are lucky if a dr will talk to you more than 10 mins. I understand not wanting to do the ADT therapy route but if that is the therapy needed right now, it is something to consider. In his book, Dr Myers recommends tri-med hormone therapy. I think its a personal choice depending on whether you are open to alternative medicine, doctor's orders or both. There is information online about alternative medicine for prostate cancer through Google. Like I said, its personal preference. Are you Greek? Since you live in Greece half the time? If you are, O Theos mazi sou!!

Thank you Hope and Prayer. I was born in an extremely poor neighborhood in Athens, left Greece when I was 17 years old. Both of my grant fathers were major tobacco farmer in Smirny , or Smyrna in Turkish. We lost everything in 1922, including my two grandfathers. I live in Canada and Greece and have Business in Canada, US and Greece.
I called and left messages with Dr Myers but haven’t heard from his office yet. Take care of yourself. George
