Question for Sling owners. Can you feel it?

I'm at 15 months post DaVinci and am still incontinent at about 120ml per day. When I am active it gets up to 300ml. I am perfectly continent when sitting, in bed, and the morning until about 10AM, when my scrotum starts to descend.
I had the cystoscopy done and was told I have a short bladder sphincter. When urinating, I can stop and start my flow without trouble.

From my admittedly limited research, at my leak rate it seems the sling is an iffy proposition for me. However, the incontinence specialist (who specializes in slings) thinks it is the right choice for me. Naturally.
I prefer a sling to an AUS. But I would like to know beforehand if it would work.

This brings me to my question. When you push up in your perineum can you feel the sling? When I push there now I will squirt out a little urine, like dumping out the last bit in my bladder. Can you estimate how far above your perineum the sling sits?

Hi Jeff,
As you know, I can't be of much help since my sling is MIA. But, I can feel the cuff for my AUS. I am guessing it it somewhere in the upper third of the perineum closer to the bladder. Probably not any help to your question. That said, I am still a happy camper with my AUS and life is definitely back to normal. Still wearing a light panty liner during the day to catch any residual drops after I pee. I can go anywhere and do anyghing with out worry. Including play a full 18 holes of golf without changing the panty liner.
After all of this I am convinced that I would prefer the AUS with more total control than a sling that would still leak a modest amount under stress. I just don't find that happening with my AUS. Somewhat interesting to note that I still have a little numbness (numbnuts) of the scrotum after three months that seems to be getting a little less each week.
The only downside I think of with the AUS is the possiblity of needing to replace it in 5-10 years. But then, that's just another operation and after three in two years I think I can handle that. That would put me at 71-76 so odds are I may get by with only one or two replacements depending on how long it lasts and how long I last.
For what it's worth as you work on your decision.

Incontinence & ED. Dry nights , no control during day.
FUDS and cystourethroscopy testing 10/09.
24 hr pad wt. 219gm. AUA sympton index 13/35=moderate
Virtue sling surgery 2/3/10 NYU med center
Appeared to help but gradually returned to pre surgery condition.
Back to 3-4 pads/day.
Latest PSA 4/1/10 negative.
AMS 800 artificial urinary sphincter implanted 7/8/10 at U of MI.
AMS 800 activated 8/25/10 Dry for first time in 2 years.

Thanks for getting back to me. I'm glad your AUS is working for you. You are proof that there is life after incontinence.

Obviously I am on the fence here: AUS vs. Sling. At 15 months, I know have to do something. My existing leak rate is manageable if I only "tiptoe through the tulips" all day. But life is more than sitting still. Just one draw on my Bowtech Diamond Liberty and my pad is full - even with the 80% let-off cam. Bend over to throw a log in the fire and I get a small squirt running down over my bits and into the pad. Jump up from the kitchen table suddenly to answer the phone and I have to continue the run to the bathroom. (Hey Cajun Jeff, Next time you call, listen carefully for the tinkling.)
The incontinence doc is a sling man, University of Rochester Dr. Mayer, so naturally he will recommend the sling. (Or am I just being a cynic.)

Does anyone here with a sling feel the cling or is it so far up inside as to be out of reach? Can you simulate the action of the sling by pushing up on your perineum or is it a completely different action?

Unfortunately, I know there are no guarantees in this business but I sure wish I could at least try before I buy.

I am starting to wonder if there are no sling guys currently active on HW. Otherwise someone should have answered by now.
You have heard all my stuff but as I think more about where you are I still reflect on my past and say that I am glad I tried the sling even thought it did not work for me. Only down side is six weeks or so of recovery and the possibility of two operations. If you and/or your insurance don't mind the sling is always worth a try...my opinion. At least this will bump this thread up again in case a sling guy is listening.

Either there are not many guys with slings or every one who has a sling is satisfied and doesn't need to come back here.
I sure thought it was pretty common. I guess not.
I am still hopeful somebody will answer.
