This is just a rhetorical question since I am already in line to get an artifical urinary sphincter(AUS). Ok, i had the urodynamics test which, as I understand it, measures the amount of pressure that my bladder can hold before it leaks. That concept seems easy for me to grasp, the bladder starts filling and at some point the sphincter can't hold it. So, after I empty my bladder seemingly very little pressure would be present but as I walk around with a basically empty bladder I dribble. So if my bladder is leaking only because of inablity to hold pressure then it should not leak until then was enough urine to create pressure? The fact I dribble with very little or no pressure makes me wonder if my sphincter is not just weak but deformed somehow? THe urodynamics was followed by cystoscopy and there was nothing amiss but a very short sphincter.
I don't see how any RP can leave anybody continent since the surgeon must join a thich muscular organ ,bladder, to a small tube ,urethra. It seems to me that the surgical connection for the urethra stub of the sphincter to the bladder after the neck is cut off would be like sewing your pants waist band to the cuff of the pant leg.
I wish I didn't sit around thinking about things like this since I doesn't make any difference but I think I might get some peace of mind if I could visualize the area. Or maybe not. I wish I had thought about this when I was being tested, so I guess I'll ask as I am being wheeled down the hall to the operating room....... say doc, explain to me about the sphincter......

That's a very good question.
Wish I could come up with an answer, but soon oters will chime in and educate both of us.

Hey knotreel,
You and I are in a similar situation.
I did urodynamics testing on myself. I determined my bladder pressure by measuring how far my pee shoots when squirted from a known height (my inseam) and a known angle (horizontal). I made an Excel program that does this. I put marked nails in the pea gravel (named "pee gravel" by Goodlife) in the garden so I could measure distance and watched my pressure increase from about 2" H2O to about 10" H2O for the first 5 months after surgery until it stopped. I then calculated the size of the orifice that would cause a leak of 120 ml per day over a 12 hour period. I'll save you the trouble - 72 um, 0.072 millimeters! The size of a thick hair. My sphincter is short, too. The cystoscopy showed it to be about 1 cm.
I have control when I sit and when I am in bed and in the early morning otherwise I leak. I worry that the planned sling won't work for me.
I hope this helps you.

I never thought about doing a self urodynamics!! I can tell you that the test I had looked fairly complicated, a tiny uro cath a rectile cath and a number of electrodes stuck on various spots , all this while sitting in a big chair that was fashioned after a toilet seat design with a funnel under all this was hooked to a computer that produced various plots. I didn't get any numbers but it was clear that the answer was go past sling to AUS.
What you came up with was a constant opening that drips which is what I was thinking, otherwise I would not start leaking till I had an inch or two or more of head pressure. I leak just like you, sitting laying down pretty much dry but constant while on my feet. I have not peed even one time in the last 11 months while on my feet. Dr. didn't say how long the sphincter was but that it was short. He ran the camera back and fourth a few times across the sphincter so i could see. My guess it might even be shorter than a cm.
He said the AUS results are unpredicable and would not venture a guess on how much better I would be, but that I would be better, just how much is a wait and see.
Well I hope your sling works!! I even though I am not expecting to be dry I am hoping.
Damm this leaking. I am still in the 0.0 club psa wise but in the 2% club leaking wise.
Ron who also leaks

I dribbled after urinating since the age of 30 (now 68) surgery seems to have cured me of that problem. My docs always said some people just do and not to worry.
