Incontinence and Bladder Pressure

Each week brings a new experience, or at least a modified one.  This may be normal (if there is such a thing) but it is new to me.  I've been doing my Kegels and progressing in the incontinence department.  What I've noticed the past couple of weeks is if I have a real full bladder, I leak less. 
 
This morning I was working at my desk and until the pain got to me, I was busy so ignored the sensation.  I got up, walked to the restroom and emptied without incident.  No leakage on the way.  On the return from the restroom, I leaked.  This seems backwards to me?

The pressure on the whats its name valve was greater thus keeping it closed??  (just made that up - but it sounds good)

I've noticed the same thing, when I have some pressure reminding my brain to keep things tight no leaks, when empty I get an occasional drop.

The sphincter that was controlled by involuntary muscles is gone, it's the one that was kept closed 'till you consciously relaxed it, the one that is left your brain has to continually keep tight even though you may not be thinking about it at the time (sub-conscious takes care of it) so when there is no pressure the brain may slack off a bit leading to a drop.

At least that's the way it was explained to me.

I've had this same problem for over a year. Full bladder is no problem however, I occasionally leak one or 2 drops when I just emptied my bladder.Regardless of my kegels and exercise I haven't been able to rid my self of the 1 or 2 drops on some days. I can live with that since it doesn't require wearing a pad.
Age: 67 at Dx on 12/30/08 PSA 3.8
2 cores out of 12 were positive Gleason (4+4)
Davinci surgery 2/9/09 Gleason 4+4 EPE,
Margins clear, nerve bundles removed
Prostate weighed 57 grams 10-20% involved
all PSA tests since (2, 5, 8, 11, 15, 18 months) undetectable
Latest PSA test (21 months) .005

Hey Pocketman

Very astute observation. I found the same thing. To learn continence I would drink a big glass of water before bed and could actually make it through the night. I was so proud of my morning deposit into the bowl I wanted to invite my friends over to watch! Unfortunately after that morning pee it was all downhill from there.
I have fingers (and knees) crossed hoping this surgery will fix it.

Didn't realize my brain was even involved in this situation. (Although I've been accused of thinking with that part of the anatomy.) Perhaps this is related to training oneself to urinate on a schedule? I appreciate the replies knowing now that my experience is "normal."

I've had the same thing since a couple of weeks out of surgery, I leak when I'm empty but can hold it easily when my bladder is full. It only fills if I'm sitting or lying down. My continence therapist didn't understand this. I'm down to 1 pad a day so I'm assuming new muscles are slowly providing more control. open RP May 2010.

It seems that when you have been for a pee there is a certain amount of urine left in the uretha, this needs to be coaxed out by gently sroking the scotum upwards towards the penis.
When you were younger this was not a problem, a quick shake and all was dry, but as you grow older the muscle tone goes  and without the prostate and the sphincter at the bladder neck some work is required to get rid of the last few drops.
I would think that this is part of the problem you are referring to. The stroking works for me and I have been told by my prostate nurse that this is what she suggests to her patients.

Tuesday, prior to getting the AUS implanted I was asking my Dr. about something similiar and he refered to the description given by Dogbot. He added that some guys have a little dip in the uretha just outside of our remaining sphincter and a little urine can pool there after urination and drip out as we walk.

Before surgery I leaked a few drips. This had been going on for 30 years. Now I don't always leak after going but when I get up from sitting I have to remind myself to keep it closed or I leak. The colder weather here in Florida seems to also be having an effect. I leak more when the temps are below 50.
I don't want to hear from you people up north about cold. I used to live in upstate NY so i know cold. I came here to SO Florida to be warm. 45 is not warm any more.
