4 1/2 years post op - trying to get erections sufficient for intercourse. I've tried everything, the latest being tri mix. It worked a couple of times, but even with increased dosages to .40, the results are spotty, with some aching.

Am wondering whether to gradually increase doses and continue working with it, or consider an implant.

My wife is understanding, we're both orgasmic through oral, but I don't really want to give up intercourse yet, at age 56.

A bit discouraged temporarily, any ideas gratefully appreciated.

DW,
I am almost 2 years out of surgery and in the same situation. Have not tried trimix yet due to some ongoing medical problems. Keep the faith and hopefully a corner will be turned somehwere down the road. I am enjoying making love with my wife, but have had to find others ways to do it.

I'm 2.5 years post-op, and erections are difficult to achieve. They come and go. I've assumed they won't return and am surprised when, suddenly, the occur.
Age: 61 (58 at diagnosis - June, 2008)
April '08 PSA 4.8 ("free PSA" 7.9), up from 3.5 year prior
June '08 had biopsy, 2 days later told results positive but in less than 1% of sample (Gleason's 3+3=6)
Developed sepsis 2 days post-biopsy, seriously ill in hospital for 3 days
Dr. recommended robotic removal using da Vinci; Surgery 9/10/08
Northwestern Memorial Hospital, Chicago, IL; Dr. Robert Nadler, Urologist/Surgeon
Post-Op Gleason's: 3+3, Tertiary 4; Margins: Free ; Bladder & Urethral: Free
Seminal vesicles: Not involved; Lymphatic/Vascular Invasion: Not involved
Tumor: T2c; location: Bilateral; Volume: 20%; Catheter: Removed 12-days after surgery
Incontinent: Yes (1 to 2 light (woman's style) pad per day)
ED: Combination of Cialis and MUSE (alprostadil) once weekly: started 9-27-08
Returned to work 9-29-08 (18-19 days post-op)
PSA test result, post-op, 10/08: 0.0; 12/08: 0.0; 4/09: 0.0; 9/09: 0.0; 3/10: 0.0; 9/10: 0.0

I'm 2 years 4 months post op. Went the the usual pump, pills, and prick. I must say the injections work very well. There are are some misses with the injections but not that often. I agree with you 100% were way too young to give up on the pleasures of SEX.

The next adventure is implant. Many guys say they wish they done it sooner. I recomend that you visit frantak.org that site deals specificly with ED. Lots of info.

Hang in there, yeah I know the problem is too much hanging. There are things to get you back in the ball game.

With injections, technique makes a great deal of difference. I too had very erratic results early on, and as a result of suggestions here, tried constriction at the time of injection and a few minutes afterwards. Made a big difference.

Other here get good results pre-plumping before injections.

I suggest don't give up on trimix until you thoroughly test both methods. They appear often in injection threads -- if you have no luck in finding the details, I'm sure we can...
No symptoms; PSA 5.7; Gleason 4+5=9; cancer in 4/12 cores
Non-nerve-sparing RRP 7 March 2008 age 63
Organ confined, neg margins. Gleason downgrade 4+4=8
Fully continent
Bimix worked well; now using just VED
PSA undetectable at first but now 0.4, doubling time 7 months
No radiation but ADT coming unless I can slow down the rise...

dw, here's an outtake from a post detailing injections. This method woorks for almost everyone who tries it. See if this will work for you:

As far as technique, its simple, actually. Use the least diameter needle, usually a 5/16" 31 ga. insulin syringe, and inject into the penis midway between the head of the penis and the body, at the 9 or 3 o'clock position. A little milking motion of the penis will help open the cavernous cavities a little, so the drug cane be deposited into the space. A little massage and the erection should begin, usually within 5 to 10 minutes, and if enough drug is injected and adsorbed, results in a penis erect for an hour or so. That's the idea goal. It can vary. Done right, with practice, the injection can and will be painless for most guys. There's no reason to think you would be any different. I don't combine by injections with other items such as pills or pump, but some here do. I, and several other guys, use what I call a pre-plump method to ensure successful injections. It begins with a penis pump, if you have one, and a penis ring. Using the pump, obtain a partial- maybe 30%- erection and apply the ring. This engorges the penis with blood, opening the cavernous cavities and filling them with blood. Then inject into this expanded cavity. Imagine injecting into an uninflated balloon, and putting the drug into the inside, versus injecting into a partially inflated one. That's what is being accomplished with this method. When injecting, you can easily tell when the needle point enters the cavity, as the resistance will drop and the needle will feel like it has 'fallen' into it. Inject and very lightly massage the penis, the purpose is to spread the drug around and ensure optimium adsorption of the drug. Keeping the ring on for 3 or 4 minutes holds the blood (and drug) inside and lets it have the best chance of being adsorbed. Continue to gently massage the penis and the drug around for 3 to 4 minutes, then remove the ring- the blood will most likely run back out of the penis, until the erection begins. You should get the drug induced erection in 5 minutes or so. Most of us who use this method find we can use much less drug, we can control results more consistently from one time to the next and reduce the chance of priaprisms.

I tried the injections 10 years ago (50 yrs old when RP)  and many things have changed since then reading everyone's response on dialing in etc.

I have been using the VED ( vacuum pump) with good results.
It took some time to get the hang of it and it isn't as spontaneous but both my wife and I have satisfactory organisms using my penis.
Some of the problems which I have had and solutions
1) Pumping up, I can make it too hard and then putting on rings, I don't have the feeling in the penis - it is desenitized
2) Using the wrong size rings or too many - desentized or erection goes down.
3) Scotal tissue and the vacuum can be a problem - the bag gets sucked in and you put on the ring - resulting in little pain.
4) Too much lubricant on pump - will affect what gets sucked in
5) I was incontinent - with AUS, that should help, and couldn't pump up too soon or had leaking.
6) You need to practice - your penis needs blood, the more you practice the easier it is to get a firm erection - you don't need to have intercourse just pump up and get
    blood flowing.

7) You need to realize that the 18 year old erection isn't what you have when you are 45, 50, 60 or more.  Naturally it will not be the same but it doesn't have to be.
You can satisfy your wife/partner with less.   It is more the action of love making than the erection.
8) The quickie ain't what it use to be, you need to take your time so both your partner and you can enjoy each other - Quality not Bam thank  you Ma'm.

If you wife went through the entire process and you talked about it.  She knows what the outcome maybe.

You are not half the man because your Penis isn't what it used to be.

When I was young, my Dad said don't let your Penis think for you.  Now I am old and use my mind to make by Penis think what I want.

Good Luck - your wife sounds like a winner to go this far with you.

Keep it up !!!  tongue
