Just had my tests last week, waiting on Biopsy.  I'm 48, my overall PSA is 2.9 - been climbing at about .5 for the last few years, and my free PSA is 3.4% - so I know there's a good chance where this will go, and a 40ish % chance it wont.

Just wondering if you guys have heard of anyone with free PSA that low?  I see posts all the time with fPSA levels of around 5% to 7%, just don't seem to see them as low as mine.  Is that any indication of agressiveness?  That's really my bigger concern, I'm actually ok from the reading I've done and will come up with a plan, just hoping it's not overly agressive and I've got it early.

I've been on propecia for over 10 years, so I know my numbers can be off, my uro says he would really double the PSA, it's probably higher with propecia, but at the low dose, probably not double.  I also don't have any other symptoms - I will say from time to time something doesn't exactly feel completely right, but that could just be my mind.

Thanks again for letting me ramble - just feels better the more I know going in wink

Welcome to HW, sorry for why you are here. I think most of your questions will be answered with the biopsy results. I don't know the details of the freePSA to offer anything, but some may come along who do. It's slow on the weekends, so it may take a day or so.

Hi gadget, Welcome to Healing Well. I do not know the answer to your question, but my Gleason score was 4+5=9, and my fpsa was 18%. I do not recall ever seeing a correlation between fpsa and Gleason score.

I do recall that, generally, a fpsa over 25% was a good indicator, and under 15%, was a negative indicator. That suggests that between 15 and 25% is a gray area.

Having said all that, my impression is that there is no relationship between fpsa score and Gleason.

Hope that helps a bit.

Biopsy 8 of 12 positive, Gleason 3+4, T2a
DaVinci August 2009, pathology Gleason 4+3, neg margins, T2c
Continent right away, ED
Viagra, Cialis did't work, Trimix works well
Post-surgery PSA:

Thanks guys - I've done a lot of searching and did find one guy that appeared to be down to 4% fPSA and his biopsy was still neg - so I guess there's still a chance that may be my case too. I'm really probably ok either way - just being young @ 48 (I'm sure we all think we're too young :)) I'm hoping that if it is positive, that it's caught early. That's really my bigger fear. Seems like there's a lot of options and the cure rate is pretty darn good if it hasn't spread.

My PSADT, from what I can tell, is at like 35mos - so it doesn't sound like it's moving fast. Even if I double my PSA due to Propecia (which again my Uro says he really wouldn't double???) - I've been taking it so long that all my readings are "under the influence" so it really doesn't change the velocity or PSADT - just MAY increase the PSA number.

This all seems almost more art than science - I really wish you could just plug in your #'s and know, yes, you're positive or no, your negative, but I guess we don't live in that world yet.

gadget, this is an interesting thread, and I'm glad you posted the question. Please let us all know how your biopsy goes, as well as your results.

I wish you well, and pray for a negative biopsy.
