(I was treated in the Netherlands so many things were different to the US system)
I saw first one uro and then another, but not specifically to get a second opinion, but because the first uro did not do Da Vinci robot surgery. The second surgeon was at a different hospital, but said almost the exact same things as the first, I think there are basically national guidelines and protocols they follow.
However, when I was talking to the first uro before a decsion had been made, while he did recommend surgery, he also said he would be more than happy to put me in touch with a radiotherapy doc if I wanted RT instead.
He also told me I should go away and not make any decison for about
a fortnight, but spend the time investigating things, and that when I came back even that decision didn't have to be final and I could always change my mind. (I could have changed my mind right up till the day of surgery!)
And I basically left his office with a "Dear Colleague" referral letter explaining my DRE, PSA, biopsy results etc which I could take to any other doctor or hospital in the country.
As for questions I reckon there are basically two categories, the first and at this stage the important ones about
how many times has he done the surgery? what are his short medium and long term results like for patients of similar age Gleason etc? Will it be possible to do nerve-sparing surgery?
The second type of questions are relevant once a final decision is made and are related to the days before and after surgery, and are about
Do I need to lose weight get fitter? etc
When do I have to stop eating and drinking or take laxatives?

There is more atwww.healingwell.com/community/default.aspx?f=35&m=1717641
but there is quite a lot covered in that post.
Write down the questions write down the answers and make sure you can phone up to ask any additonal questions. (I made a point of saying that I had decided to ask a lot of questions even if I thought I knew the answer, or even if I thought they might not be relevant, as I wanted to feel like I had asked one question too many rather than one too few.)
All the best
