"Staging" before treatment is very subjective. The things you mention about test and scan results are much more important than that number.
I would add, in regard to lymph nodes, that you discuss this directly with the surgeon if surgery is chosen. There is no real standard as to if or how many might be taken, as there are some risks for every added bit of surgery.
I will also suggest strongly that he get a consultation with one or more radiation oncologists, to discuss either radiation or seeds, or a combination. They were not valid for me, but they may offer advantages in long term side effects. In order to make this decision, he should be comfortable with why he rejects the options he does not choose.
He should be tasked to read Walsh front to back. There are also books by those who oppose surgery, and read in context, will give some points of view that should be considered.
No one here can or should suggest a specific treatment based on the rules of the forum. We can only comment on our research and decisions.
And lastly, the side effects must be understood and considered. When they say 9x% will be continent within x months - I'm part of that other %. It wasn't supposed to be that way, but it is. For me.
He has to understand them, even if he hopes to not experience them.
