Despite our past differences i must say that I admire the fact that you are trying to help your friend by providing him with education and a balanced approach. The links/info provided are good. The only thing I might add is to suggest a visit to www. dattoli.com. While this web site may be self serving to the doctor's practice (I guess most of them are!), it does provide the added perspective for which you are searching.
BTW, it's not only in Italy that docs try to rush patients into treatment. While I was seeking additional opinions one surgeon, after looking at my test results but with no physical exam, wanted to schedule me for surgery later that same week because "an opening just came up". He even walked me out to his scheduler to get me signed up. Can you say, "Doctor needs a new pair of shoes!".
Anyway, good luck and please let us know what happens.
