I got Duke's number from their website and set up an "all day" appointment, where I met with a urologist, medical oncologist and Radiation Oncologist. They were impressive and do it all I believe.
I ultimately stayed at home (Atlanta) as I found a RO that I was very confident in here. Had I not, that's where I planned to go. I lived in Wilmington, NC in the early 00's, btw.
Family History: Father - prostate, colon, intestinal, bladder, brain, and melonoma cancer. He's 81 and still alive.
PSA: 2013 - 2.5, Aug. 2014 - 4.5, Oct. - 4.0
3+3=6 (<5% of core), 3+4=7 (10% of one core), 3+4=7 ("Discontinuously" involving two cores, 40% and 50%) Left base, mid and apex.
