Newt, based on the g8(4+4) you are not eligible for active Surviellance, you will need treatment,
First thing I would do is get a second opinion on the slides fro Dr Jonathan Epstein at Johns Hopkins. You never know, I was downgraded on second opinion from g8 to g6.
Assuming the second opinion confirms the first, and your Scans come back clean, you'll consider surgery or one f the many forms of radiation .
Plan on meeting with more than one surgeon, and more than one Radiation Oncologist. There are different types of radiation, as you probably know from reading the forum, and you should find experts in each of them.
Don't rush into anything too quickly, take the time to educate yourself but don't take TOO much time, with a G8.
The good news is it appears there was no cancer found at the apex, which is an area that has a higher chance of escape of cancer from the prostate capsule.
The positive perineal invasion may not be the best thing, but it's controversial, as I understand it, as to whether it means much. Others more knowledgeable than me will likely address that
And maintain your sense of humor, it will come in handy.
