I've had 3T MPMRI with endo-rectal coil, CT, and repeat 3T MPMRI scans throughout the years at various different medical institutions. CT scan and MPMRI are standard procedure for most radiation protocols from what I've experienced and seen. I'm not as familiar with what scans are standard protocol for RALP though.
Push back on the insurance company - it can't hurt.
