Since you've already had surgery and radiation, HT is the next step. The real question is when to start it. HT does have a limited time, but the limit is a few years. Then once HT fails, there are a number of other treatments that can keep you going for a very long time. At this point you're colon issue sounds like a more urgent thing to check into.
One of the problems with your current PSA is that it's so low, scans won't show where it is in your body and likely it's not causing any symptoms that are bothering you. That means that treatment now isn't that helpful. Often we hear of guys in your situation waiting for the PSA to rise to something above 3 or 4 where a CT or other scan might find out where it is. Then you have a choice of just monitoring, trying local RT on the mets or jumping on HT. This lets you avoid getting on HT too soon and might buy a few years.
It's a delaying action, but during the delay you have a better quality of life.
