Sadly, his best shot at the moment is to get back on HT. The side effects aren't nice, but neither is allowing the PC to spread. At this point an oncologist is likely to offer HT and when it gets bad enough, chemo of various kinds.
The hope is that with proper treatment, they can keep the beast at bay for many, many years. We have a number of guys here who've been gone, 5, 8, plus years and some are still going.
There are plenty of options and even with his high numbers there are things they can do. Don't lay down and do nothing. Keep fighting.
He still has lots of ammo left.
