On the east coast, my "top" list would include Howard Scher at MSKCC, Emmanuel Antonarakis at Johns Hopkins, Andrew Armstrong at Duke, Richard Lee at Mass General, and Elizabeth Plimack at Fox Chase. Heading west from you, Sartor at Tulane, and Logothetis' group at MD Anderson come to mind. All of those have expanded the envelope in treatments and are apt to work with you to explore new ones. I think every major tertiary care facility has excellent urologic oncologists on staff.
