Halbert, to your point, and as has been stated here in the past, there are many great doctors, that are not among the well known elite, and even that are not practicing at the large NCCN centers. Someone. Said, maybe here, I don't recall, a doctor, for example, may be one of the best, but chooses not to practice in mManhattan, or Chicago, because he or she doesn't want that lifestyle for themselves.
The problem is, those doctors don't get the press or publicity that the "big name" doctors do, so it's harder for a layman to judge he competence.
I think the best you can do is look for a doctor with many years of experience, practicing at a higher volume facility. Doesn't have to be Tewari, Moran, Eastham.
