Germany (particularly U of Heidelberg) has really led the way on this. The US is just starting to get on board, particularly Scott Tagawa at Weill Cornell. The trial in Houston is mega-expensive. MSKCC is running an interesting trial now too.
If it were me, I'd consider medical tourism. The guys at Heidelberg have more expertise than anyone else.
