(You will also notice there that the toxicity of focal salvage brachy doesn't seem to be too bad, so far)
One of the problems with salvage ablation is the incomplete ablation that can happen. I know that you are painfully aware of this in your own case.:/pcnrv.blogspot.com/2016/12/focal-ablation-unresolved-issues.html
The Ahmed/Emberton group in the UK have the most experience with salvage focal ablation. In the US, you can try Gary Onik or Jonathan Coleman at MSK.
For focal salvage brachytherapy, my top choice would probably be Joe Hsu at UCSF. I'll be discussing it for a patient with Dr Demanes this month - I don't know how much experience he's had.
