They usually take 12 cores on a TRUS biopsy - 2 from each of the 6 sectors of the peripheral zone (base, mid, & apex - on the R & L). Each core is 1-2 cm long, and they usually do not
penetrate through the capsule, unless there is some reason to (e.g., suspicion of EPE). A few pathologists (like Epstein or Bostwick) will report if there is a core where cancer tissue is mixed in with prostate bed tissue, or if capsular tissue intervenes, but this is rarely done.
