Thanks, guys. Yes, according to my plan everything involving payments (other than from my checkbook) must go through Kaiser. Since my initial post I've spoken with the people at both testing companies. For Decipher they'll give me a great deal: $2500. Oncotype is potentially more interesting in that they have a payment appeals process that if it comes down to Kaiser refusing to reimburse them, and me "assisting" with said process, my final out of pocket expense would be only $200. I'm certainly pursuing this.
As far as treatment goes... well, since Dr. Bahn as well as a second Kaiser urologist have both come down on the side of surveillance, I'm hoping to be able to do so. Going for yet a third opinion next week to try and either seal the deal or confuse me even more.
I'm definitely not interested in surgery and don't feel it's warranted. If you twisted my arm and said I needed to treat tomorrow, I'd look seriously at a focal therapy like HIFU or FLA, and keep SBRT in my back pocket as a fallback. Two of the guys in my local support group have had this last and come through quite well.
