So my dad's liver enzymes increased 3 months post taking zytiga - they were at ~1.5 times the normal limits. The MO advised him to lower the dose to 3 tablets (750 mg) along with 5 mg of prednisone and get the liver function tested in 15 days. The liver enzymes got back to the normal range. His last PSA was 0.103 tested on November 06, 2019. Since we have a ~2.5 months long trip planned to australia in 10 days time, the MO said to continue the lower dose of 3 tablets of zytiga for 3 months - when i expressed concern if it would be effective in keeping the cancer at bay- he asked us to take these 3 tablets with food (high fat food such as ghee) - he said you should take them with lunch or breakfast - when asked if it would be fine and wont increase or magnify the dose to higher than what is required- he should no it wont be a problem. Studies i can find online state taking 250 mg with low fat meals such as yoghurt have been effective. Some are comparative between low fat and high fat.
The MO did state that once we are back - he will recheck the liver n psa numbers and then we will go back to 1000 mg without food. But for 3 months, we should stick to 750 mg. Now, I am getting worried if taking 750 mg with food should be fine? or should he take 750 without food (but scared if that wont be enough to control the cancer) or should we continue to take 1000 mg and pay out of pocket and get liver numbers tested in australia? Anyone has experience of taking zytiga higher than 250 mg dose with meals?
