That is a very useful and directive message. In it, to put it briefly, he says that his best treatment is by one of the radiation methods. He's basically saying, that if you do surgery, he's going to lose both nerves--and have complete ED forever. Also, he's saying that he will likely need post surgery radiation. And then he says he could have hormone therapy for a few months, followed by radiation.
Any time a surgeon says these things..he's saying that he isn't the guy you want, and that surgery is not the best choice.
I'd run with that.
