To get back to the original question:
NEPC: Neuroendocrine Prostate Cancer.
Unfortunately, there is always a possibility that a prostate cancer cell mutates into an NEPC type cell. However, this appears to be rare and I doubt that you will find a solution to the question you raised for two reasons:
1. We don't know where the prostate cancer loci are as of today.
2. Your last (PSMA) scans are two years old by now, and didn't reveal mets. Did your physician suggest that your cancer is 'hiding' in the prostate bed? If that were the case, perhaps further study might be warranted.
For the time being though, I suggest that you live life fully (but distance yourself appropriately) and not worry about
the scenario that you described.
