Adjuvent Radiation questions from the wife

I would like to know from all that have had to have more treatment after surgery?  Did all go on hormone therapy prior to radiation? Is this the protocol?  Also on the treatment planning and radiation, how often is the Radiologist Oncologist involved? Or is it mainly administered by the technicians?

Radiation Oncs do all the planning and overseeing of treatment. Treatments are done by techs with the Doc in charge. He'll see the Doc at least once a week during treatment...good luck.

Hormones for two months prior to radiation are recommended to weaken any pc cells before they get zapped.  Some have continued with the hormones for 6 months, while my docs would like me to continue for up to two years.  I guess the longer, up to two years, the better.  After a CT scan and directional tatoo dots applied, your radiation oncologist will design the treatment physics.  This was one big charge that I had to re-bill the insurance coverage - they didn't understand that design was needed.  The technicians will handle the treatments, each one only about 20 minutes total.  You get to know them all like friends - they even gave me a certificate of completion at the end.  I also saw the radiologist once a week, but this usually was very quick.   I had very minimal side effects from the radiation.  I get some "warm" flashes from the hormones, but very little "libido" now.  However, I'll take this to conquer this beast.

Hawk and I are parallels. I front ended the IMRT by a month and a half with Lupron. I had no insurance issues though. I am still on Lupron and Casodex. Might stop in January. I miss the staff. They are like friends. We had a breakfast club of sorts. I was the "second of the three good attitude guys". We were there at 7:00am and out by 8:00. I have wine once in a while with them as they too are now friends. Saw the RO almost daily as I was there when he would arrive for the day. But we met for business once a week. The rest of the time we would talk about his three Porsche's or my hiking locations. He is an avid hiker as well and we still communicate that stuff. The will likely be a breeze for you.

Surgery on Feb 16, 2007
Post-Op Pathology was poor: Gleason 4+3=7, 4 positive margins, Stage pT3b (Stage III)
HT began in May, '07 with Lupron and Casodex 50mg
IMRT radiation for 38 Treatments ending August 3, '07
 
My PSA did drop out after surgery to undetectable.  It has not returned and I will continue HT until January '08.

Hey mjr,

After reading Hawkfan75's response I cannot express any better the par allel that his radiation and mine were. Were I going to post on this topic that is exactly what I would have said. Also Tony's pc and mine are very close in all respects and we have almost had identical treatment. I started HT in late June and then started RT in mid September (35 treatments) I had not one problem with the RT and have had very mild problems with the HT. Good luck in your journey. JWB

positive margin left side of prostate
left seminal vesicle involved (both removed)
No lymph node involvement
New gleason score 9
T3B....radiation consult on 5-9-07.
June 5, 2007 1st psa post surgery 0.62
June 27, 2007 second psa post surgery 0.59
June 27, 2007 started Lupron injection (24 mos)
September 6, 2007 scheduled to be marked and prepped for radiation therapy.
Start IMRT therapy on September 13, 2007.
September 26,2007 psa test .08
Finished IMRT therapy October 31!!!
