Elevated TPO is present in about 95% of people with Hashi's and about 50-60% of people with Graves. Low TSH and high free T4 are associated with hyperthyroidism, which would be Graves disease.
The caveat is that people with Hashi's can have wide-ranging test results, and sometimes even edge into the hyperthyroid range. If you aren't seeing an endocrinologist, that would be a good idea. Be sure to find one experienced in treating thyroid problems, as some are really only experienced with diabetes.
With elevated TPO and normal hormone levels in the absence of symptoms, many endo's will elect to follow and watch rather than treating. This is because there are other factors that can temporarily affect TPO.
