high thyroglobulin Ab (TgAb) low thyroid peroxidase Ab (TPOAb)

Hi all, I just did a thyroid ultrasound today and I got back results for my previous thyroid antibody tests.

Thyroid panel (2 weeks ago):
TSH: <0.005 mIU/L (0.510-4.30),
Free T4: 45.5 pmol/L (12.5-21.0)

Antibody test results:
Thyrotropin receptor antibodies (TRAbs): negative (normal)
Thyroid Peroxidase Ab (TPOAb): L <5 IU/mL (5-34)
Thyroglobulin Ab (TgAb): H 217 IU/mL (10-115)
Thyroid stimulating immunoglobulin (TSI): *still awaiting results

Other tests:
Erythrocyte Sedimentation Rate: 4 mm/hr (1-10)

From the results above, do you think I have an autoimmune disease i.e. Graves disease , I highly doubt Hashimoto because my TPOAb is normal and I have hyperthyroidism and hyper symptoms, have lost significant weight and feel hungry all the time. I also have hypertension and my current medications are methimazole, propanalol and amlodipine.

Hi,
Your TGab is high which indicate hashimoto's disease, with hashi, one can develop both hypo and hyper, in your case it is hyper as indicated by your low TSH and high FT4 .

The propanalol is to reduce your heart rate, as hyper affects the heart very badly.

Obviously , the Methimazole does not work for you(unless you are on it for not enough time, how much ?) so your doctor might recommend RAI, after taking RAI treatment you will eventually become constantly hypo and will need to be put on Levothyroxine for life.
