You need to give the thyroid replacement medication a few weeks (about six) for it to make an impact. Do the research. As far as fasting goes, try to get an early morning appointment for a blood draw. Make your last meal one that contains fewer carbs, normal amount of protein and plenty of fat (e.g. Salad with olive oil, avocado, olives, bacon, etc). That will help keep you blood sugar steady.
