I started taking Prednisone (20 mg) for the first time about two weeks ago. Up until a week ago, I was having about 10 loose BMs a day with urgency, blood, and mucous. My doctor said that once I'd had one "good week" to call him and we'd start tapering. I'm not sure if this past week has been good or just...different.

I now have a BM every other day, a small amount that comes out like large-ish pebbles. I have less urgency, but the BMs are difficult to get out and I pass almost no gas. I still have some blood and mucous. Also, I'm still pretty (visibly) bloated. I feel like I'm constipated. Does that mean I'm getting better, or is it just a different variety of problem?

I feel like my diet could be to blame as I've been eating a lot of cheese and junk food lately (woops). I'm just not sure if the Prednisone is working or not. Should I taper, stay at this dosage, or ask for something more? I'm so confused...

For what it's worth, the only side effects I've noticed are tiredness and "brain fog," which make focusing at work a challenge, but it's better than it could be.

I have major constipation issues when flaring due to the inflammation. To me it doesn't sound like you are ready to taper especially if you are still having blood. I do think you should call your GI and give him an update.

It took a long time for prednisone to work for me and I think I needed cipro and hydrocortisone enema's to finally jumpstart my remission.

I started pred 60mg a day on 9/7/07 and stopped asacol cold turkey the same day
I went into a huge flare end of Sept/Oct (probably from stopping asacol cold turkey)
In Oct started cipro (2 week course) and hydrocortisone e's (one month course)
in two weeks began to get better

Currently I am on 60/40mg of pred alt days and have had no blood in almost one month (before that it had been two weeks and I think that was just from the hemi). My stools are still stringy but no mucous and no blood. I am gassy but I think that is from the fiber cereal (am stopping that now).

HTH!

Don't forget to drink alot of water with the prednisone - it actually helps you not retain as much water and stay away from the cheese if your worried about weight gain !! or any other high salt food. Not that I should talk as I am sitting here eating tuna fish salad with bread( which has alot of salt). I think that drinking water might help with being constipated.

So, I just spoke to my doctor (mentioning everything I said above) and he basically said that constipation is good and I should start taper to 15 mg and call him next week. I'm happy to trust the Dr., I just hope it works. I've read about people having side effects when they taper - is that something I should expect (despite my minimal side effects and low dose)?

"Constipation is good." Since when is trading one abnormal bowel function for another a good thing?

I'd advise you to increase your probiotic supplementation. You may also want to look into a fiber supplement. Both would help normalize your bowel functions.
