Whilst I'm tapering off oral prednisone, my GI specialist has adviced me to:

Crush methylprednisone tablets, mix with a lanolin based substance, and apply rectally.  I will also be taking 1 canasa to help push the mixture further up.  I've been begining to flare slightly after tapering oral prednisone down to 20mg; and we're trying to address this rectally instead of increasing the oral dosage again.

Has anyone tried this?  Any recommendations as to which lanolin based substance worked well for you and what's the best way to create the mixture and apply it?

Thanks.

Alternative therapies for several months (SCD, ayurvedic treatment, VSL#3): didn't work and became anaemic. Canasa, 2 daily: remission for a month then flared-up.
07/07-11/07: 4800mg Asacol (6 x 400mg, twice a day) & Aloe Elite.
Completed first (6 week) tapered course of Pred in 10/07, flaring up since coming off it.
11/07: Switched from Asacol to 12 x 750mg Colazal (3 weeks on it and symptoms got worse)
12/07: Asacol 4800 mg, Started Azathioprene 175mg, Prednisone 60mg - taper.

He wants to try something different to see whether I can respond to this combination instead of increasing oral steroids.  I don't think he's tried this before, but it sounds like a reasonable effort and, at worst, in a few weeks time I'll increase the oral steroids if this doesn't work.

We've had quite a few people on Cortisone enemas or Cortifoam, but I don't think I've ever heard of this. Let us know how it works out for you, Rio.

What's the reason your doctor isn't prescribing the rectal steroid preparations (foam/liquid/suppositories?)

I was going to ask Quincy's question, but he got there first.I've never needed a pestle and mortar for my treatments!Steroid enemas and suppositories can be very effective, especially for distal iflammation.

Getting a pestle and mortar out and creating the mixture does sound a bit too radical.  I'm holding off from doing that and actually began using Pentasa suppositories yesterday.  I've tried cortifoam in the past and it only provided very temporary relief.

Rio,do you know how extensive your UC is?Rectal meds will only extend up as far as 6-7 inches. They can stil be effective in conjunction with oral meds, but it sounds like you've been trying that.I'm still not sure of the possible benefit of what your GI has suggested. However,he is a professional and it's possible he's tried it before, with success.I'd have another chat before trying it,but maybe you'll find out it's got some validity.

Either way,I hope you can find something that eases things soon.

Silent Lucidity - thanks,

I have left-sided colitis.  A sigmoidoscopy in December '07 showed it extending as far up as the scope went. (I haven't had a colonoscopy yet and have that scheduled for this April).

In the past two days, the Pentasa has actually stopped any visible bleeding and I'm having formed BMs so it seems to have worked for now.  I'll begin tapering oral prednisone below 20mg next week and see how things go.

Rio,not to be bleak, but it sounds like the left sided diagnosis comes from the lack of a C-Scope.As you know,a Sigmoidoscopy goes no further than the Sigmoid Colon(not to belittle the procedure,it's not pleasant!).A Colonoscopy would confirm if it really is left sided,or if it's more widely spread.

I'm sorry to say though, that your UC extends too far for rectal meds alone. However, please bear in mind, that they do help in all variations of the disease. To my knowledge, the rectum is always affected,so obviously, rectal meds will help with that part of the equation.

It's been my experience,to just keep going.By that,I mean not to stop a certain med, because you think it isn't helping. In the last couple of years,I've had flares that took three months to control. Not much to some here, but a lot for me.Just be as obstinate as you can.If you get intolerable side effects from a certain med, fair enough. But other than that, just keep taking it/them.

Somewhere along the line, you should catch a break and nail the ******!
