Prednisone Tapering

My son has tapered down to 20 mgs. of Prednisone from 40 mgs. over the past 3 weeks.  So far, so good.  Actually so far, very great :)  Can I stop worrying about the possible side effects, or is it possible things can start going badly? He has had no problems at all.  He is going 1-2 times per day, instead of 20. Looks great, feels great.  He is going to try 6mp, as soon as the results of a blood test come back.  Asacol seemed to have stopped working. Any thoughts?

The first time I used prednisone last year I felt the side-effects up till a few weeks after I had tapered all the way down to zero.
Also, once I had tapered below 20mg things got a bit shaky (literally, I got shaky hands - but also had minor flare-ups) so I'm tapering at a slower rate below 20mg this time.  I'm on a second course of prednisone at the moment and have tapering down to 15mg. 
 
A few people suffer no side-effects, so hopefully your son's one of them.  More importantly it's great that he seems to be in remission or headed in the right direction.
Rio, 31 yr old male - diagnosed with UC in 07/2006.
 
Alternative therapies for several months (SCD, ayurvedic treatment, VSL#3): didn't work and became anaemic. Canasa, 2 daily: remission for a month then flared-up.
07/07-11/07: 4800mg Asacol (6 x 400mg, twice a day) & Aloe Elite.
Completed first (6 week) tapered course of Pred in 10/07, flaring up since coming off it.
11/07: Switched from Asacol to 12 x 750mg Colazal (3 weeks on it and symptoms got worse)
12/07: Asacol 4800 mg, Started Azathioprene 175mg, Prednisone taper from 60mg - Now on alternate days of 20mg and 10mg; 2/08: added 1 Pentasa x 1mg.

Thanks Rio. We switched GI's and the new one has slowed the taper down (2 weeks on each increment), so I hope he can manage without side effects. After his first flare and diagnosis, he moved into remission in about 3 months with Asacol. This one has been more stubborn and the Asacol was not helping.

Prednisone tapering can be tricky. It takes a few days for your body to be at the actual level that you are at. And the longer you are on them the more side effects you will get. I normaly have started haveing problems when dropping to around 20 or 15. But also I get less side effects. But I will go into a mini-flare and will be held at around 20 or 15 for longer. I have gone into a flare both time while trying to tapper off of the prednisone. So I have disscused Remicade with my GI as the next step if I can not get of off prednisone with this tapper (i'm at 25 so keep your fingers crossed.) I was diagnosed as a teenage as well, so I know how hard it must be for your son. I drasticaly changed my eatting habits, and with the prednisone side effects i just never felt like myself. I hope that everything goes well and he can get off of the prednisone sucessfuly!

meds: Asacol 4 pills 3x a day; 6-mp 75mg MWF, 50mg SSTTH; iron supplament; protonix; prednisone 25mg for one more week! (in the middle of a tapper, down from 40mg started 1/1/08)

Good luck to you too!!  I feel like he is very lucky so far, and I am just hoping the luck will continue. He has dropped in 10 mg. increments, with zero problems.  It has worked like a miracle. Take care of yourself!

Typically when tapering down from 20, you want to decrease the dosage every 5-7 days by 2.5 mg. You want to split the dosage equally between the morning and evening dosage. Decrease 2.5 mg at the nightly dosage. When the night dosage reaches 0, then split the dosage again. So when you have 10 mg in the morning and 0 mg at night, the next day you split 5 and 5. I read this online, it is called the Marshall Protocol. This is how I tapered down, but your GI may have other ideas.

I am down to my last 2.5 mg, and will be off of this by next Monday!!
