Hello all... I'm still dealing with my almost year long flare now. Waiting for insurance to kick in so I can start Remicade.

Anyway I've noticed lately (past few days) that when I have my morning BM I'm really straining and there is a lot of pain. Does anyone else experience this? Throughout this entire flare I've dealt with urgency, mucus, bleeding but never any pain. I've tried all the rectal meds, cortenema was working for a while but I can no longer retain it overnight and it really bothers me. It did work great for about 3 weeks though.

Hopefully it won't be to much longer before I start the Remicade, and I really hope that turns out to be my wonderdrug....
29 year old male, diagnosed after bad flare in Summer 2005, quickly brought into remission by pred and salofalk. 2 years symptom free after that!!
Currently in a flare since summer 07 - I desperately miss remission!!!

Current Meds: finally off Pred (after months of 40mg), 2000mg salofalk, calcium supplements, Pantoloc, Buscopan, Clonazopam, Cortenema
Started 100mg Imuran Jan 15/08 - no noticeable effect yet... aside from nausea, vomiting, fatigue...

I hope that Remi will be your miracle drug too! :) Yes, the chief complaint that I have is always the pain. Definate pain upon defecation - I compare it to pooping razor blades! Also the abdominal pain like aliens trying to rip out! Yes, I'm big on using metaphors lol In regards to the steroid enemas - have you tried just using a half a bottle - until eventually you would be able to retain the whole bottle? Have you tried warming it up?

Yeah I've tried all the tricks with the enemas, half bottle, warming etc... nothing seems to work anymore. It's so weird a few weeks ago I could take the whole bottle over night with no problem, now I'm lucky if I can last an hour.

Also throughout the day I'm getting the urgency to run to the bathroom but when I sit down it's like I'm constipated. Which is really strange because I have never been constripated a day in my life. Anyway, I'm trying for force out stool and it makes me feel like I'm going to throw up. Has anyone every experienced anything like this? I called my GI but the earliest I can see him is Apr 29th and unfortunately he didn't really have any suggestions for me aside from watch what I'm eating and take it easy (great advice I know).

Yes, I have been going through that right now :( and it's strange because I am eating a pretty gentle diet. The pain is so intense but as soon as I go to the bathroom it goes away. Also 2 days ago when I was struggling to go I almost vomited. It sounds like you are in a bad flare with an inflamed rectum, that's why you can't retain the enema. I don't know what the abdominal pain is, maybe inflammation in the colon as well? I haven't had these abdominal cramps in years. When I was having them, I had ulcers in my colon. Now I am wondering if they are coming back and that's the horrible pain. Inflamed colon walls mixed with stool=pain?!? I hope you can get the Remicade soon, my insurance took forever at first, I had to kindly remind everyone to keep up on my case till finally it got approved. I hope it works a miracle for you.
Diagnosed with Left-sided UC in 1995 at age 15, Prednisone, Rowasa & Hydrocort. Enemas, Proctofoam, Sulfasalazine, Asacol, Probiotics, Fish Oil, Aloe Juice, Canasa, Enotcort, Colazol, Anamantle, Remicade, etc... had 1 inch deep rectal ulcer 2004, put on Remicade, August 2007:Increased Remicade dosage-700mg every 6 weeks,diagnosed w/ Psoriatic Arthritis & Fibromyalgia Current meds: Clorazepate, Lomotil,  Tylenol pm, Lyrica started 1/17/08, Flagyl 750 mgs started 4/2/08, Humira started 4/4/08

Like Red, I get pretty lyrical when describing the pain. I used to wonder how that barbed wire that had worked its way through my gut all day suddenly turned into molten lava when it came out. The pain is quite unique with UC, I think.

Asacol, Rowasa, Pentasa, Prednisone, Entocort, Azathioprine
Colazal,  Remicade, Nature's Way Primadophilus Reuteri. In remission since April, 2006.

I had a bad pain after BM's but it was accompanied by fever. I ended up going to ER with a bad infection. Do you have a fever? My most recent flare had a lot of urgency...but I couldn't 'go.' Turned out I had inflamed rectum...
Diagnosed with Crohns/Colitis in 2004. Currently taking Azulfidine 500 mg/3 x 3 day, Synthroid. Have had joint complications such as pyroderma gangrenosum in ankles as complication of IBD.

You might want to give fibre supplements a go, my GI told me to take them daily for the rest of my life and it's helped huge as I had the same issues you're describing, not so much anymore though...probiotics are a benefit as well.

My bum is broken....there's a big crack down the middle of it!  LOL  :)

OH yes,  I hate this the worst when I am flaring...it is so painful.  I used to say it was like my body was trying like mad to push everything in my body out at that moment, but nothing comes except the blood and mucous.    The medical term for it is "tenesmus".  I was so glad when I found that there was an actual medical term for it,  it validated what I was experiencing.      Its when I get the tenesmus that I break down and admit I am flaring and call my GI.    i"m so sorry you are experiencing this!

                                         Medications
Remicade infusions ~ 11/6/07, 12/6/07, 12/20/07, 2/14/08,  6-MP 100mg daily, Pentasa 500mg twice a day, prednisone 20mg 10mg 5mg daily, Nexium 40mg twice a day, Lantus insulin 45 35 25 20 units daily, Novolog 10 8 5 units with sliding scale at meals, Amaryl 2mg daily, multi-vitamin daily, Calcium daily, B complex vit daily,  and pro-biotics daily.

Numb...where exactly is your UC? When you do go...what comes out?

Can you not take any oral 5ASA meds?

When you insert the enema....do you put the nozzle all the way in and then squeeze?

Can you possibly start the Salofalk enemas again? or mix them with possibly a cortifoam instead of the liquid?

~multi-digestive enzymes as needed ....zymactive 3 - 5x daily
~Ranitidine,Pariet (reflux) Effexor XR 75mg;  Pulmicort/Airomir (asthma)
~URSO for PSC (or PBC) 500mg X 2 daily (LFTs back to NORMAL!!)
My doc's logic.. "TREAT (FROM)BOTH ENDS"  worth it !!!

Or another suggestion - you can try the steroid suppositories to get the inflammation down in the rectum as much as possible and once the inflammation is down, revert back to the enemas?

This is a symptom that I get with every flare. I can't believe my colon hasn't be pushed out yet. lol.

I am currently flaring (as I do every spring like clockwork) and have found 500mg of Salofalk suppositories have help tremendously to stop this.
 
 
Dx:  UC Proctitis 2006
 
Meds:  None so far. Garlic works to ease flares. My GI laughed when I told him and said it was just coincidence.
 
Started Meds:  Apr 9 08 500mg 5ASA (salofalk) to ease flare.
 
Diet:  Regular fresh garlic, Biobest yogurt daily, Omega 3 supplements, very limited junk food, carbs and processed food, low fat diet.  Lots of fresh fruit and veggies (limited potatoes). 
 
Added: tumeric and probiotics.

The only time I ever had pain like that, it turned out to be a rectal abscess. It is unusual for people with UC to get abscesses (unlike with Crohns) but it does happen. I hope your GI can see you soon.
