Bleeding on at the tail end???

I have been in kind of a mini-flair on and off for the last month, no D some abdominal pain, extra gas and a bit of blood on my stool but only stripped and only on the very last piece or two (the first part of the BM is perfect). Since I switched to Lialda about a month ago I asked my doc if it's the Lialda and he up'd my dose to the max 4 pills 2xs per day and increased my rowasas to every night.
 
I asked him since I'm only getting blood at the tail end of a BM is that indicative of upper bleeding? He said it could still be rectal; but since I have no blood on the paper etc, leads me to believe it may be in fact upper bleeding and the timed release action of the Lialda is not working for me as well as the good ole Asacol. Anyone have any thoughts or experience with this?

Hello bbc,
I havent tried Asacol or Lialda but I do experience some bleeding, only when I have gas. If I have a BM theres no blood, but sometimes I feel like I have the D and when I go to the bathroom, I just end up having lots of gas and when I wipe theres a little blood. It doesnt happen all the time but it happened today..kinda scary since Im still getting used to having UC. I just got diagnosed about three months ago. Sorry I couldnt help with any advice =[

I would say stay on the Rowasa for a few weeks and then start to taper them once the bleeding has stopped.  Let the med do its thing....seems you're in a good situation and that the Lialda is working since your symptoms are not indicative of higher in the rectum bleeding).

When you insert the nozzle, start squeezing some just at the opening of the anus then continue to squeeze as you push it in, then back again...it'll fill up the rectum with the med to get a concentration of it at the lowest part of the rectum.

Hang tough...the lowest part of the rectum is the hardest to heal. The reason being is that you'll usually have more formed stool going through.

Thanks guys.

My question is since there is blood only on the last couple of pieces of stool and the first few are completely clean is this indicative of bleeding higher up or rectal? (My GI doc said it could be either way but I only feel pain in my upper stomach area). BTW, the blood is lightly stripped on the inside of the stool and lightly on the outside too.

All I know is the higher up the bleeding, probably the darker the blood in your BMs.  I dont think it means that you are bleeding higher up necessarily just because it is on the last to come out.  If you didn't have UC, I would think the blood could be from hemmroids/ it still may be.

Hi roxZ..the bleeding is from my UC for sure as I have been getting abdominal pain too and it all started about 4 days after I switched from Asacol to Lialda; plus I am taking 4 Lialda now (4.8 grams) vs. 3.2 grams of Asacol. I just don't know if it's Lialda's special release machanism not working well with me or that I 've been super stressed out due to moving...If I am not better by wed or Thursday the Dr wants me back on Asacol...The blood color is a medium red...

This happens to me too sometimes. This is my theory - don't know if it holds true or not. But when you are flaring, your colon is in a fragile state. You don't always have to have "ulceration" in order for it to be a flare. The mucousal layer if compromised it can break open a bit more easier. So when the stool comes down it may scrape the area causing it to bleed a bit and then it gets downloaded to the rectum in which of course the stool sits there until you expel it. But keep in mind that we usually flare from the rectum and it spreads up. So from the rectum to higher it can be in a fragile state. So either A) the bleeding is higher up or B) the blood can be old blood because blood when it sits for a while darkens. The brighter red blood is usually inidicative of bleeding closer to the rectum.
