Starting Canasa suppositories....

Hi everyone,
I am new here and hope someone here can help me and reassure me.
I have UC - left sided but very low in colon - mostly rectal.  My gastro doc gave me Canasa suppositories to try first for the ulceration.
I have used it twice - 1000mg. two nights in a row.  No bad side effects but still a lot of bleeding and diarrhea as before.  How long before I should get results AND can it get worse before getting better?
I am 68 and lived with IBS for many years before having bleeding.  Had colonoscopy/endoscopy last Wednesday.

I was using canasa when I was bleeding and had severe inflammation. It worked very quickly for me. I think I noticed getting better in a day or two but I can't remeber exactly. I wish I could be of more help. Good luck to you. Bellski

Hi..welcome to the forum!

Left-sided UC would definitely be past the sigmoid and up into the descending colon...did the doc say your UC is left-sided?

Are you also on any oral 5ASA med such as Asacol?

The Canasa will take at least a few weeks for you to notice a lot of improvement....use them nightly.

If you don't improve any over the next week or two, call the doc and get him to either up the dosage to twice daily or to prescribe you retention enemas.

Hi!

Obviously, I am really new at this but not really.  I have had IBS for many years but one year ago developed UC (doc just called it that) but it cleared up without medication - I only took Imodium occasionally as with IBS. This time I want to have knee replaement surgery and surgeon will not do it until I can clear up the bleeding - for obvious reasons.

Colonoscopy/endoscopy last week and was told (and he showed me on model) only in rectum and tiny bit up left side of colon.  He said really mild but causing some bleeding and usual IBS symptoms.  Not much pain or cramping.

Doc said try suppositories 3 weeks and if they don't work I believe he will give me oral medication next. I cannot use the enemas due to my obesity, etc.

Thanks for your info.  I was just wondering if Canasa could make it worse before getting better.  I am VERY sensitive to all medication so I think that why he is trying this first.  Actually, I have been living with this for months with not much problem - just take Imodium when needed.

P.S.  I added my medical info so maybe that helps.

I do not know or understand most of the terms here or part of my anatomy - sorry.  I will keep trying what the gastro doc tells me.  I have researched colitis some.  I am happy to say I don't seem to have very bad symptoms.  Imodium controls the diarrhea - just hope to get rid of the bleeding so I can have knee replacement.

If the supps don't work, the doc should add the oral 5ASA....but keep you on the supps as well.

You can also do the supps up to 3 times a day if you need.

Hope you feel better soon as to get your surgery.

I've taken Canasa as needed for a few years now and what I've noticed is that when I insert it before bedtime, when I expel it in the morning if it's done its job the remnants will come out in a dark, almost blackish-purple color.  If it is coming out the same color as when it went in, it's really not having much of an effect (at least for me).  It seems to work best when you can retain it overnight.

Have only used three so far - seems to be coming out waxy like mucous pieces.  Would the purple be the medication?  Maybe that is what I think is blood (although I know some is)  I do keep it in all night.
So far no change either way - still  having blood, diarrhea and cramping but only mornings for short time.  Just wondering how long I should try before calling doctor - he is extremely hard to get to on phone.  He told my husband and I that I should notice a difference in one week.
Also like to know if anyone has gotten worse using this before getting better.

Since you're having diarrhea, the inflammation is higher than the Canasa can deal with (my perspective). You should really also be on an oral 5ASA such as Asacol.

Call the doc by Wednesday at least.

Why can you not do the enemas based on your weight? You mean for insertion? I think they'd at least be better for you to deal with things more quickly.

It's good you're not getting any worse, however.

I also do the Canasa suppositories during a flare, and I do think they take some time to take effect (for me). My most recent flare landed me in the hospital for a lovely week's stay, and I did the Canasa twice a day, which I think really helped.

As for diarrhea - I have had that for twenty years - bouts of it diagnosed as IBS - blood is coming from rectum only so doc said. I NEVER have blood after the first couple stools in the morning. I have controlled the diarrhea with Imodium for years - not taking any until Wednesday if I have to go out.

AND - little embarrassing but I cannot do the enemas because I am too obese to reach that area and have arthritis bad so makes it impossible. I can barely reach for the suppositories laying on my right side. Have to move quickly and painfully.

I will call doc by Thursday (day he is in his office) if I don't get any relief at all.  Today has been good - nothing but one bout this morning - less blood and less cramping.  I believe Asacol is what the doctor menntioned if Canasa does not work.  He knows I am very senstive to medication and I think being conservative to start with.  He has 20 years experience especially with seniors so I am trusting his judgement.  Thanks.

UPDATE on Canasa suppositories.

I have been using 1000mg. every night for 10 days.  The bleeding seems to be stopped - 2 days ago.  BUT, I had such urgency last Thursday had to take an Imodium to finish shopping.  No BM on Friday - little on Saturday and had to take Imodium on Sunday to take a 100 miles trip.

Small BM today expelled rest of suppository but no bleeding and no cramping.  Left side pain seems gone - just a little lower back discomfort (sitting in a car four hours can do that too). 

Hoping that the suppositories are working.  They certainly are a "pain" in the butt.  I have a hard time keeping them in and have to concentrate in bed for about an hour.  But worth it if they work.

By the way, doctor or anyone from his office never returned my call.

Glad to hear you have some improvement. As far as the doctor calling is concerned. Here is my advice. I am not an aggressive personality. But, you have to keep on them. Call every day or twice a day. Ask for the nurse, the doctor, anyone. When the recording says press ---, try different ones and leave messages with several people. Sometimes you have to be a squeaky wheel to get what you want. You can be nice, you don't ever want to be mean or it may backfire on you. Just be persistant as all heck. lol.....Try it and let me know if it works. Bellski

Hi...you could try using the Canasa twice a day. That might help.

Regarding holding it in...try to push it up as high as you can...higher than where the anus ends.
Wet the tip of the suppository with water before inserting.

Use them for a few weeks....seems they're helping, however...

How unfortunate the doctor hasn't called back....sigh. Definitely call back daily. That's really rude of them....unless someone forgot?

Thanks from everyone here.  Love this website - makes me feel better just to know I am not alone - even at age 68.  Most of you are younger I know.
I took Imodium for two days because I had to be out of the house, etc. - all was good - seemed better.  Then today with no Imodium for over a day there was blood again but the stool is more formed.  Not much cramping.  Maybe it is working but not up far enough.  Who knows.
As for pushing the suppository up further - I can't due to my arms aren't long enough (due to fat and not the length ha ha).  I could not use it twice a day unless I got back to bed for an hour at least - it would come out.  It makes me gassy for one thing.
I do believe after 11 days on Canasa that there is tiny bit improvement and maybe I am one of those who will have to use it longer.  I am giving it more days unless it gets worse.  I have to take Imodium when going out however and that is what I had the question about to the doctor.
Tomorrow is VA hospital with my husband - long ride and I have to be able to not have to stop.

My doctor told me to use a little ky on the suppository tip. It works like a charm.

I do use a water soluble lubricant but can't reach far enough to get it as far as I would like. 

Wednesday seeing the Nurse Practicioner and hoping to get something to treat from the other end too.  This is working but not enough.
