Does anyone do the opposite of prednisone taper?

Why do doctors start patients from 0 to maximum prednisone? I guess b/c patients don't feel any ill effects from the sudden change in meds and it gets in there ASAP for quick relief, but wouldn't it make sense to work up to that amount. I guess prescriptions are never ever prescribed in that manner. I am just curious.

Well, I had terrible headaches, lightheadness, and some "roid rage" (just ask my husband!) when starting pred. I'm desperately trying to taper completely off of it now. I'm getting close. I've been on it almost 2 months and I don't think it helped.
Diagnosed with UC 1992

In remission with no drugs 1998-2001
Remission taking Asacol, 4/day, 2001-2008

Current flare started 3/08
Asacol 9/day
prednisone 5mg/day (desperately trying to get off!)

colostrum, 480mg, 9/day
Natren probiotics

aha, I had asked the same question to myself.

I flared @ 5mgs of pred on 5/13/08 (not sure if aza is faling me or coz I took aspirin a couple days before)
I put myself up to 10mg's for a couple of weeks, not much difference, then I put myself up to 20 mg's a couple of weeks, not much difference, but when I went up to 40mg's (and called my GI a week later, MY BAD, he said was ok) I was doing much much better in about a week and a half and right now I am doing well 2 weeks later, I think I am nearing 'pred induced remission'.

However, it could be if I stayed on 10 or 20 for a month I would have gotten the same result, but methinks it is b/c the 40mg helped me and if I ever flare again and have to go on pred, I want to see if I can just start at 40mg and see how long that takes, but also I think each flare I respond differently........I digress, my theory now is that the 10 and 20 did not help me get better (kept me from getting worse) but the 40 is what I think helped.

Azathioprine 200mg 1xday nightly; Calcium and Vit D 500mg 3xday, Multi Vit, Folic Acid 400mg 2xday, Prilosec, Probiotics.
Minimal Change Disease (Kidney Disorder) dx'd 09/07 - partial remission since 03/08
Prednisone 40mg 1xday, Simvastatin 20mg 1xday, Diovan 160mg 2xday. Tekturna 150mg 1xday, Fosomax 70mg 1xweek. MCD may be from hypersensitivty to 5ASA drugs.  Do you have edema? If so, check your blood protien level!

My doc starts pred at the high level because at that level it works in four hour. Why wait four weeks for relief when you can feel good in four hours. She won't let me stay at a high dose though once relief is established we go down quickly to where I start to have breakthrough symptoms.
