faxed 160 pages to Chron's and Colitis center

they said they wanted my medical records faxed.  All my GI records, Kidney and Primary.
 
will call to confirm they got the info.

cover letter:

Dear XX,

I would like to make an appointment with a Gastroenterologist.  Enclosed are my medical records.  At this time I am on azathioprine while tapering prednisone.  I am not sure if the azathioprine is working for me and my next step seems to be remicade.  I flared at 5mg of prednisone last taper (may be b/c I took 2 aspirin, don’t know for sure), then went back up to 40mg and tapered, today am at 20mg.  At this time I am not taking 5ASA’s because they may have caused my kidney disease.  Also, because of my kidney disease My GI and Nephrologist may switch me to Cellcept instead of Azathioprine.

 

I would like to consult with one of your Gastroenterologists, because I want to understand more about Cellcept for ulcerative colitis (avoid flaring) and if azathioprine fails me and remicade does, my next option may be surgery.

 

If I can provide any additional information please let me know.

UC dx'd 03/00 (Proctosigmoiditis); Pancolitis since 09/07
Azathioprine 200mg 1xday nightly; Calcium and Vit D 500mg 3xday, Multi Vit, Folic Acid 400mg 2xday, Prilosec, Probiotics.
Minimal Change Disease (Kidney Disorder) dx'd 09/07 - partial remission since 03/08
Prednisone 25mg 1xday, Simvastatin 20mg 1xday, Diovan 160mg 2xday. Tekturna 300mg 1xday, Fosomax 70mg 1xweek. MCD may be from hypersensitivty to 5ASA drugs.  Do you have edema? If so, check your blood protien level!

thanks, it could be worse, I keep telling myself I could be living in a cardboard box in a 3rd world country with no running water, or next to someone in a box, wishing I had my own box with UC. I am very lucky in a lot of other ways.

I'm in a weird mood today........just feel like hey, it's all just going to be all right.

fish oil, flaxseed oil, bee pollen/royal jelly & garlic, calcium & vitamin d 2x a day, prenatal vitamin plus extra vitamin c and b12 complex

Good luck, I hope they had paper enough in their fax . . .can you just imagine them scrambling around to get all the papers coming out and then trying to get more paper in while your fax is still coming through.

I like to put silly thoughts onto things, it helps keep me sain sometimes!

I hope they had enough paper!!!  Every time I called they kept on saying to fax it.

WishfulMA - I am about 45 min north of Boston just over the NH border.  I saw the "MA" and was wondering if you were MA too.

Yup - I live about 15 miles west of Boston. I just had a second opinion at Beth Israel w/ Dr. Mark Peppercorn. Good luck with your appt. I am new to the boards...would love to chat offline...how do you exchange info?

i am just curious about this, but could you not call them, or did they require all that before they would see you?
