I recently had a Barium MRI, an Camera into my stomach, and another Flexi Sig. The GI Profressor said there was nothing wrong and that I should come off all medication including thr Asacol. He said the previous Colitis wasn't UC as there was no marks on my colon plus the biopsies were negative then and now.
 
They did find a stomach ulcer which was causing black stool but that has now cleared up, God willing. And they said I had slight positive C-Dif which they gave me antibiotics for.
 
But in the past week or so my bowel movements have increased to 4 times a day. I have IBS and small piles which the doctors say is causing bleeding and the frequency.
 
The problem is IBS plus Piles have the same symptoms as Colitis, Chrohns and even Cancer!
 
How do I know that's  not what it is though? My piles are so tiny they could hardly find them, and I haven't bled in ages and now it's bleeding. On the tissue, on the pan, dripping and coating the stools - often making them darker. This started this afternoon and the anxiety is playing havoc with my IBS.
 
I only had those procedures above a month ago so they probably won't listen. It's now 3 different top private GI's (I live in the UK) who keep saying I am fine.. but why do I get the nagging feeling that something is wrong? Why does blood make me freak out?

You'll probably have to go off the meds and get retested to see if your bleeding gets worse.

When you are bleeding, the doc can do a look-see to confirm hemorrhoids.

You don't have a fissure do you?

I wish I had a fissure, I wish I had bleeding piles, I wish someone would just tell me this for a fact. Each time they look they don't see the piles bleeding, in fact they can't even find them but they say the smaller piles bleed more. I have had 3 top GI's look at my colon and none can conclude UC, two have confirmed that ISN'T UC and one is sitting on the fence.

Nobody can explain the frequency and urgency + blood. The only answers I have are IBS+Piles - I have no pain, no weight loss or anything like that. Just anxiety and a dodgy stomach.

I just emailed all three of them now but I bet they say "don't worry about it, it's probably piles"...

They say the Barium MRI has ruled out Chrohns.
The Colonoscopy has ruled out Chrohns and UC but not colitis with a small c.
The two sigmoidoscopies only found piles.
The X-rays, MRI and the above ruled out the Big C.

But I still bleed and have diarrhoea from time to time and urgency. Surely it can't be as simple as IBS and Piles?

So, your last c-scope was with LOTS of biopsies?

Have the docs done a series of marker blood tests? To determine inflammatory conditions, autoimmune, cancer?

Is your urine still dark?

How often are you having bms?

Do you have any bms that are just blood and mucus?

Are you having mucus?

I think you should TAPER off the Asacol to see if your symptoms improve or get worse.

I know it's easy to get fixated on the fact that there could be something else going on....it's always possible since some things don't show up until they're more obvious....but your distress about it will truly make things worse.

Did the docs do a small bowel series?

Did they check for microscopic colitis?

Are you taking any other meds or supplements? which ones if you are?

So, your last c-scope was with LOTS of biopsies? They took 100's of biopsies last time. From all over my sigmoid colon. Previous to that he took loads in a colonoscopy. Nothing came back as confirmed IBD.

They did 2 blood tests a day over 5 days I spent in private hospital - everything normal. One of the docs said that the blood was healthier than healthy!

Is your urine still dark?
No

How often are you having bms?
Normally one or two a day, maximum three. Maybe 4 or 5 when I have heavy anxiety.

I don't get mucous. Sometimes a little blood comes out on it's own but usually after a stool.

I think you should TAPER off the Asacol to see if your symptoms improve or get worse. One of my GI's told me to cut down the Asacol to twice a day. I did this for ONE day and then I got this problem. I rang him and he said it's impossible to go from 6 tablets to 4 in just a day and notice anything different. He said the blood is from anxiety and IBS.

Did the docs do a small bowel series? They did a small bowel MRI with a drink and dye.

Did they check for microscopic colitis? They have checked for everything.

I took Omeprazole for the ulcer which made the diarrhiea worse but I don't need to take that anymore as it's healed,God willing. I also take a probiotic - biocare.

I'm sorry you're having to deal with all of this. I totally understand that seeing blood in your stools can freak you out. It's not "normal" to see blood coming from there, so it's totally a normal reaction to feel frustrated and anxious like you do. But...try to take a deep breath. If 3 top GIs told you that you don't have UC or Crohn's, that is a good thing! I wouldn't worry about cancer if you've had all those tests done recently. The GIs would have seen something (almost certainly) if that was the case. Why not try to be happy that you don't have UC or Crohn's and can get off medication? I agree with your GI that it would be highly unlikely for you to get worsening symptoms after one day of going from 6 to 4 Asacol tablets.

I just re-read your last post. I'm not sure why your doctor would tell you the bleeding is from IBS since IBS does NOT cause bleeding. Have they tested you for celiac disease? It's a blood test.

Thanks Sara. He said that going from 6 to 4 tablets is insignificant even over a few weeks for mild inflammation. He also said he did a scope only a month ago and there is "no chance" it can get worse like that so soon. It all makes sense but doesn't explain the blood.

I will admit the IBS, the urgency and regularity is linked to my anxiety but the blood? It just makes no sense. Plus if it wasn't UC then why did the blood stop after taking Flagyl and Asacol last October?

I am trying to use common sense. I am sick of being fobbed off and told not to worry.

I just read you second post, Sara he isn't saying it is IBS, he is saying the IBS is causing my piles to bleed through irritation. I have been tested for Cealic too - negative.

I guess it's possible that your butt is bleeding from hemmies caused by irritation from IBS....

I'd suggest you request proctocedyl suppositories to help heal them. If the bleeding stops, it's your hemmies.

Are you at least taking fibre supplements and eating properly?

You aren't taking any artificial sweeteners are you?

my uc started the same exact way. They told me IBS and I said how can it be with blood and urgency and not making it to the bathroom. They did a c-scope again they said I see nothing wrong come back in 10 years and it got worse. FIRST AND FOREMOST DON'T STRESS YOURSELF OUT. Do whatever you have to do to remain calm, seriously. My fear made me more sick. Try to stay away from glute and dairy for a while, just fresh steamed veggies and meat if you can. Your emotions will make you worse. You can try  to head it off. Pray and stay positive. I wish I didn't freak and have the fear I did, I made myself worse.

Colonoscopy & Endoscopy 7/27, Left sided colitis and near cecum (mild)
Stopped Colozal 10/1/07 was on it for 7 weeks, side effects
Working with new doctor getting stool, urine and blood tests, waiting for results. starting back with ASACOL  6/1/08

Okay, that makes sense. Do you not think the blood is from the piles? I'm not sure why your bleeding would have stopped from the flagyl and asacol in the past...do you think it could have been a coincidence or not? This must be frustrating for you.

I find it difficult to accept the blood is from the piles even though I have seen pictures of the piles. The piles are so small and they have never seen them bleed on scope. There are two of them, very small and internal.

Sara - the flagyl working could mean that the colitis was infective or the piles could have just gone away with Asacol as piles are just lumps of inflammation at the end of the day. Heres hoping.

Quincy - I don't need fibre, I always have soft stools.

I had two small BM's this morning (I am in UK) and I didn't have blood on tissue or toilet pan but noticed some small streaks on my stool. The latter BM's didn't show blood at all. So maybe it is clearing up, as surely a flare up wouldn't be so intermittent in the space of 24 hours.

I can accept I have IBS, but I need an explanation for the bleeding every now and again.

It sucks really, although small compared to some of the good folks on here the mental aspect of illness cannot be discounted. May God heal us all.

oh my.... i dont mean to sound mean, but i think your in an unnecessary panic mode.

why dont you stop all meds like they say and see what happens.
there is nothing else you can do short of demanding more scopes which you just had recently.

you've answered all quincy's questions with pretty much negatives, so thats good.

as for urgency and blood, you answered "Just anxiety and a dodgy stomach." ... that just may be the answer right there.
especially since you say it is not a lot of blood. maybe a tiny tear in your bumhole is what is bleeding.

im of the mind that you are very lucky indeed to be told that your "all good".
just be thankful that you dont have UC or crohns.
you just posted today, give it a few days/weeks and see if the bleeding stops. relax, breathe and see what happens.
worry and stress always do a lot more damage than anything else.

I just wrote to my GI - and he has booked me in for an appointment on Friday - he still maintains that he isn't worried. Well I bloody am doctor! This is what I wrote and you will see from my comments that anybody would be worried.
 
1. I have 3 Bowel Movements a day sometimes more, regular diarrhoea - this is new for these past 2 months, normally I have one or two. Even during my initial colitis days I have never had diarrhoea or this many loose stools - where has this sudden bout come from? Could it be an infection? Shall I take anti-biotics?
 
2. Stool colour ranges from dark brown to light yellow - the dark stools make me think the ulcer has returned.
 
3. Regular stomach rumbling and bowel spasms with urgency and often violent diarrhoea.
 
4. Periodic blood on stools, on tissue and in toilet pan. Could the blood be from lowering Asacol dosage? or Could it be a flare? I don't bleed all the time just sometimes.
 
5. I think I suffer from mal-absorption - is there a cure?
 
On the face of it, it looks like IBS and Piles but both are new to me. I have never seen my piles bleed on scope and I have never really had diarrhoea although I do have stress induced IBS. When Mr C***** put me on Asacol and Flagyl the bleeding stopped - how do we explain that?
 
I am really concerned about UC and Chrohns and just want to get to the bottom of these issues. I want firstly the diarrhoea to stop and secondly to return to my normal one bowel movement a day.
 
Surely I should be worried?

Hi.....yes, I'd probably be worried especially if my symptoms continued and the docs couldn't find a cause specifically for the bleeding.

It's good he'll see you on Friday.

Regarding the stool colour...that can change with food intake as well as how fast the colon empties its contents.

He should take liver enzyme blood tests again.

malabsorption? that would happen in the small intestine, have him check you for celiac disease.

Make sure you've been checked for microscopic colitis...make sure you write this part down for a question.

Fibre supplements (I'd suggest the pills) will help bulk up the stool, slow down the colon a tad to be more rhythmic rather than spasming.

Get the doc to prescribe you dicyclomine to help lessen spasming. It'll help your IBS.

Remember, we're trying to have you keep things in perspective...you do have to calm down a bit so that your symptoms aren't confused. If you're at the top of the distressed metre, the doc will see only that....does that make sense to you? Sometimes we condition the docs into how they treat us/pacify us.....

Could your increase of D be attributed to your recent antibiotics? With antibiotics you feel great for awhile, but they can cause an eventual yeast overgrowth, thereby contributing to, among other things, D problems.

With the increase in stools, have you tested again for CDiff?  We had to treat our daughter 3x with antibiotics before it was finally gone.  Hard bacteria to beat, for sure.

I know it's hard to calm down when you're worried....I know it is for me. I freak out and come right here, but it's hard to have questions answered until we have enough information. Anxiety makes my symptoms worse. Also Asacol can cause panic attacks and anxiety. I know this first hand. I had two serious panic attacks while on the Asacol. You are taking 800mg total per day? Sometimes too much medication will make things worse. I actually just learned that myself here in the forums recently. I cut down my medication and I'm slowly on the road to recovering my worst flare ever.

That aside I would have to recommend that you keep a food diary. Know exactly what you're putting in your body and how long it takes to leave your body. Notice when your reactions are and try to find out if something is bothering you. Being tested for Celiacs will definitely help.

Reading your signature it says that they did find inflammation in your colon, but haven't called it Colitis yet?

Lots of good advice here! Please take a few deep breaths and calm yourself. The anxiety is not helping. I know, easier said than done!

I second the option of fiber supplements. They will help bulk up your stools and may help get you back to 1-2 bms per day. Fiber helps both C and D - it's finding the balance for you. Normal bms should be firm and formed - able to hold their shape in the toilet. I personally like the Fibercon brand. I know pb4 recommends Prodiem. I guess it will depend where you are at.

Hemis (piles) can bleed a lot! Fissures can too. I'll see mind dripping in the toilet after a bms when they are active.

I would suggest visiting the IBS forum and googling "help for IBS" to see if anything helps.

IBS can be very debilitating as well. It's not a which is better, which is worse scenario.

The mal-absorption issues could be IBS related as well. With IBS nutrients can FLY through the intestines and not be absorbed. The key is to slow down the spasming.

I keep getting a nagging feeling that I have Chrohns Disease but they say that the Barium MRI has ruled that out as has the Colonoscopy Biopsies. Thanks for all your help this is the help list I will take to the GI either on Friday or before if I can get an appointment.

Diarrhoea

This first started in hospital and continued ever since – could it be the Omemprazole or the C-Dificile? I have diarrhoea every day now do I need antibiotics again? Or did the Metronidazole cause a yeast overgrowth in the colon?

 

Bowel Movements

Even during my colitis days I didn’t go to the toilet this much. Normally I go once a day, now I am going 3 or 4 times.

 

Stools

My stools are all over the place, dark, light, medium – soft, diarrhoea, bloody. There is no set rhythm to them at all.

 

Stomach rumbling and spasm – do I need dicyclomine?

This has been going on since I left hospital and it means I have the violent diarrhoea and super fast transit of food.

 

Blood

The blood is on the stool, on the tissues and sometimes drips into the pan. This happens every few days and there is no set rhythm to them at all. The blood first started again in hospital in June and got worse after the sigmoidoscopy. It has now returned after a 9-month absence.

 

When Mr Chopada put me on Asacol the bleeding stopped - how do we explain that?

Could this be colitis?

Chrohns flare up?

Could it be the right hand side of the colon flaring up?

Could it be because of me reducing the Mesalazine tablets?

 

Mal Absorption

My stool colour, slight weight loss and diarrhoea suggest this.


Other concerns


IBS and Piles?

Microscopic Colitis?

Chohns? Right hand side inflammation?

Ulcerative Colitis?

Cealic Disease?

C-Dificile or other infection?

Fibre Supplements?

 

I want to come off the medication, go back to one bowel movement a day and most of all I want to stop bleeding as that freaks me out.

 

I had this only 2 months ago before the Ulcer – what’s happened since hospital?

 


Diagnosis in October 2007: Non Specific Colitis (possible UC but not confirmed) (Mild inflammation on Rectum, Plus some patchy inflammation on Right side of Colon).

Medication Still taking: 800mg Mesalazine (Asacol) 3 times a day  

Symptoms: Colitis Remission, last had black stools but it might have been something I ate, Asprin, or an infection as it has cleared up.

Just to get back on track. My D has been replaced by formed loose stools. At the end of my BM I had two small clots of blood or it could be two small faeces coated with blood. The tissue was bright red.

I'm petrified. I have set up an appointment for 6 hours time. I bet they end up scoping me again. Wish me luck. God Willing it will be okay.
