So- I've been on Remicade since January 2007 and since after my third infusion I had really great results.  Then in March 2008 I started a full blown flare- 15x a day, blood, misery- the whole UC package.  My GI puts me on prednisone (30 mg) and gives me my Remi 10 days early- then puts me back on every 8 weeks for Remi.  I manage to taper off pred, then have about a week of flaring which then mysteriously disappears, and then I was good and made it to my next Remi.  After my Remicade I made it three weeks before I began to flare, with a gradual increase in my symptoms.  At least 10x a day, blood, and I was up 2-3x per night.  My GI puts me back on prednisone (30 mg) and moves my Remi up two weeks early.

Remi this past Thursday- GI puts me on a slower taper than usual (down 5 mg every two weeks) and schedules my next Remicade for eight weeks out.  Instead of changing my Remicade to a six week schedule, he decided that my Remi dose will be changed from 5mg per kilogram to the 10 mg per kilogram dose.  My GI states that he is "optimistic" that this approach will get my UC symptoms under control and that Remicade will remain an effective med for me.  Personally, I am doubtful- I think it will only delay the inevitable- that I am done with Remicade.

My question- have other Remis experienced a dosage change on Remicade and what results good or bad did they see from the change?

I started at the 5mgs/kg dose, every 8 weeks- this was about a year & a half ago. Worked great for a little while, but was still unable to get off pred totally. We upped the dose to the 10mgs/kg every 6 weeks about 10 months ago, I last took steroids in February '08, & that dose & time schedule is still working pretty well. I say "pretty well", cause I still have urgency issues, some nocturnal bm's/gas, active disease according to last colonoscopy, plus that last week before infusions is generally pretty rough..... but we're still sticking at that dose for now. If I find myself getting symptomatic in 4-5 weeks, then I'll have to call my GI & try Humira. I feel like you do though- upping the dose & shortening the frequency is probably just delaying the inevitable. But I don't mind delaying it as long as I can. I'm in no rish ro run thru my med options right now.

Remicade-10mgs/kg- since 4/07

Thanks for the response Eva Lou- it helps to hear that you have had 10 months of benefit from the Remicade dosage change.  Also your post on your "metamucil miracle" provided me with something new to try if I flare (when I flare) after I taper off this crap pred.  Like everybody else here on this forum I am just looking for a little hope that something will work.

I have had my diagnosis for less than three years and the Remicade option seems to be winding down, I will try Humira if my insurance will cover it, and then it's surgery time.  At this point, medication just seems like a too temporary stop on my road to surgery and surgery is looking like a better option all the time.  Thanks for the hope that I can hold on a little longer on the meds.
