So tomorrow is my 2nd opinion appt, I am almost ready.
 
I am putting together 4 packets (one for me, one for the Doc, one for my Aunt and one just in case) which contain.
 
Page 1 - med list
page 2 - Brief history of my disease(s)
page 3 - list of questions
 
also secondary packet of info on my kidney disease, stuff from the internet, that describes it and has a few pic's.
 
I am going into this as a patient and as executive of Beth, Inc., I would prepare this way for any other thing going on at work so I hope the Dr. is receptive to this and I am not wasting my time.

Major Flare Sept/Oct 07
UC dx'd 03/00 (Proctosigmoiditis); Pancolitis since 09/07
Azathioprine 200mg 1xday nightly; Calcium and Vit D 500mg 3xday, Multi Vit, Folic Acid 400mg 2xday, Prilosec, Probiotics.
Minimal Change Disease (Kidney Disorder) dx'd 09/07 - partial remission since 03/08
Prednisone 10mg 1xday, Simvastatin 20mg 1xday, Diovan 160mg 2xday. Tekturna 300mg 1xday, Fosomax 70mg 1xweek. MCD may be from hypersensitivty to 5ASA drugs.  Do you have edema? If so, check your blood protien level!

Sounds like good planning, Beth. I'm getting ready to move and will have to find all new doctors. I've been around to my docs collecting records and have them in a ring binder tabbed by specialty. They can take whatever they need and copy it as I see new docs.

what a great idea Judy, that is really well planned out, I am very impressed!

I do have my records, but just in a manilla envelope, I should do the binder!

Great job both of you! I need to get that organized for my UC "experience"!!

Well....your health is your "business"...and you have a more complicated situation...so being at the ready doesn't seem like overkill..it seems efficient.

You could also ask the doctor how much time is allotted for your appointment before you start...that way he won't be antsy.

*Heather*Status: mini flare Dec 28... tapered to every 4th night
~diagnosed January 1989 UC (proctosigmoiditis)
~5ASA: Asacol (6 daily) + Salofalk enemas (increase for flares tapered to maintenance)
~Bentylol (dicyclomine) 20mg as needed
~vitamins/minerals/supplements 
~Probiotic 3(Natural Factors Protec) bedtime + 1 (Primadophilus Reuteri) occasionally
~multi-digestive enzymes as needed ....zymactive 3 - 5x daily
~Ranitidine,Pariet (reflux) Effexor XR 75mg;  Pulmicort/Airomir (asthma)
~URSO for PSC (or PBC) 500mg X 2 daily (LFTs back to NORMAL!!)
My doc's logic.. "TREAT (FROM)BOTH ENDS"  worth it !!!

I bet all doctors wish their patients would be so prepared. The docs at my gastro office have asked me questions about what meds I've taken and for how long, and I just looking at them thinking, "Look at your chart! You're the doctor!" Sheesh.

DX:  Ulcerative Proctitis (10cm) January '08
Current Treament:  Rowasa 60 ML 2X daily, Cortenama 100 ML 1X daily, Bentyl 20 MG (as needed), and multivitamin daily.
Diet/Exercise:  Specific Carbohydrate Diet (SCD) as of 8/12.  Daily yoga, walking, or cardio w/weights.
Previous Treatment:  Canasa suppositories, Cortifoam, Prednisone, Colazal, Symex DuoTabs, Vitamins.
Status:  Flare since diagnosis
