Lowest Dose of Pred?

What is the lowest dose of prednisone you've ever been started on that actually helped your symptoms?
Every time a doc puts me on pred, they always start it at 40mg at the least. I've taken it a countless amount of times, and the last time I took it, it seemed to lessen the bleeding a little but didn't help the rest of my symptoms much. I also get dependent on it and when I start to taper I flare back up in no time.
 
Problem is, my new doctor reluctantly suggested that I start on it (I don't remember the dose, it was either 40 or 60mg to start) because things are increasingly getting worse. I started Sulfasalazine and haven't noticed any change but I reeeaallly don't want to get stuck in the prednisone cycle again, so I haven't filled the script yet. Problem is, I've been feeling incredibly sick, tired, nauseous, my left side hurts 24/7, and I've been having a lot of dark blood. I know I need to do something but I don't know if prednisone is even the answer anymore.

Do you suppose it would be possible to start at 5 or even 10mg and have any effect since I didn't even respond to high doses last time? Is it even worth trying?

Current Meds:
Lexapro 10mg, Sulfasalizine 3000mg/day

Hey, i feel your pain!
I've just recently been put on the darned drug for the eighth time in 6 years. I was/am in quite a bad flare so its the only way i can get back on track... However its not something i wish to keep doing!
I to start at 5 or 10mg wouldn't kick start your recovery, however an optional alternative is to try the predfoam enemas, less of it is absorbed into the body, with less side effects and it treats the area directly!That would have been my course of action if i hadnt chanced my symptoms until i got to this stage of severe flaring...

Diagnosed in 2001,
Since then i have flared seven times, each time taking 40mg of pred for a month to manage the flare. Now on my eighth dose since diagnosis!
Currently on Asacol 6 tabs a day

I tried starting at 20mg and didn't do anything for me, so I took 40mg and began to get better.

UC dx'd 03/00 (Proctosigmoiditis); Pancolitis since 09/07
Azathioprine 200mg 1xday nightly; Calcium and Vit D 500mg 3xday, Multi Vit, Folic Acid 400mg 2xday, Prilosec, Probiotics.
Minimal Change Disease (Kidney Disorder) dx'd 09/07. Close to remission since 9/08.
Prednisone 5mg/2.5mg alt days 1xday, Simvastatin 20mg 1xday, Diovan 160mg 2xday. Tekturna 300mg 1xday, Fosomax 70mg 1xweek. MCD may be from hypersensitivty to 5ASA drugs.  Do you have edema? If so, check your blood protien level!

Bleh... :( I figure that will probably be the case with me as well. I'm just afraid I'll take it and it won't do anything but make me gain weight and give me moonface and moodswings and mess up my potassium levels like last time. And then I'll get even worse. Maybe I'm just rationalizing to avoid taking it. Ah well.

sorry I know how you feel. I have cushingoid's syndrome and have been on pred for over a year. I hear ya! I never want to take it again, ever!!!!

Aw man, I'm sorry to hear that :(
UC, MCD, and Cushings? You just can't get a break! I was always paranoid about that, always looking for signs/possible symptoms. It was like "Oh no I have moonface, could it be cushings??!"
Is there anything they can do to reverse/treat the cushingoids or is it something that just resolves after stopping the prednisone?

for me it's always been the fairly high 60mg dose... I tried to start at 40mgs once, when I was started to flare again after an entire pred taper. Was off pred for maybe a month, then boom! all the yukky symptoms came right back. So I started at 40mgs, stayed on that about 10 days. It did nothing, I literally got no noticeable relief. Granted, this was my 3rd course of steroids, & I've heard that each time you use them, they lessen in effectiveness. But I went up to 60mgs, & finally got better. It annoyed me though, because I figured if I started at 40mgs, I could get off it faster. Well, between staying at 40 for a few weeks, then shooting up to 60, I wound up on pred longer than if I had just started right at 60! My GI always goes back & forth between hitting it hard with a high dose of pred right off, or starting at a lower dose & seeing if that does the trick. With me, it's better to hit it hard with the high dose. The reason I started at 40mgs is because he (my GI) was on vacation, so I put myself on the pred. When we talked a few weeks later, he agreed it was probably the smart thing to do, but upped the dose.

     I'm not familiar with Cushing's Syndrome?  Is it something you developed from taking pred?  My friend had Addison's Disease from taking herself off the prednisone too quickly.  She was a Crohn's sufferer.

Diagnosed with ulcerative proctitis in 1998 in hospital
Hospitalized (2nd time) in May 2008
Still flaring.   Afraid that I am pred dependant.  Back on Pred 40 mgm for five days, then taper.  Other meds...Colazal, 6MP, Nexium, Cort enemas, Canasa suppository, Benicar, Toprol xl, water pill.
Calcium, Probiotic, Flaxseed oil capsule, multivitamin with Iron
Also have osteoporosis and high blood pressure
Remicade NOT an option. Not a candidate for surgery yet..says surgeon.

I must be lucky, because the highest pred dose I've had to take was 30mg. Around the same time I started using hydrocortisone enemas, so maybe it was the combo that helped. I'm working on tapering the pred because I can't stand the stuff, not that I'm thrilled with the nightly enemas either.

So sorry for all that are pred dependent.
